Medication Nurse, Full time, Part Time
Under the direction and supervision of the Clinical Supervisor, the Medication Nurse serves as the primary resource for pharmacy-related needs of participants and clinic operation. Completes data entry of medication orders, coordinates submission of refill requests, and receives medication deliveries from pharmacy vendor. Administer medication as directed in the Medication Administration policies. Assists with medication adherence monitoring as needed. Communicates with participants, caregivers, and other interdisciplinary team members about medication orders and concerns. When appropriate, provides training to participants and caregivers regarding medication management. Maintains participant medical records with thorough documentation. Participates in on-call nurse rotation as assigned.
Essential Duties and Responsibilities:
- Receives and processes medication orders daily as EveryAge PACE’s primary point of contact with pharmacy vendor staff.
- Administer medication as directed in the Medication Administration policies and procedures except injections, medication given by g-tube, j-tube or nasogastric tubes and IV medications.
- Communicates medication orders to approved back-up pharmacies as instructed and coordinates pick-up of medication from back-up pharmacy for delivery to participant.
- Maintains documentation and chain of custody for controlled medications upon receipt from pharmacy vendor until accepted by transportation staff for delivery to participant.
- Conducts periodic review of medication storage area to be sure correct inventory and order, along with a review of expiration dates.
- Assists with periodic medication cart audits when requested.
- Participates in on-call nursing coverage to troubleshoot, advise, and coordinate the scheduling of participant care outside of normal business hours.
- Assists RN Care Managers as needed with reconciliation of medication lists during participant care transitions.
Minimum Education:
Graduate of an accredited school of nursing. Current unencumbered North Carolina Licensed Practical Nurse (LPN) Licensure. Current Cardiopulmonary Resuscitation (CPR) certification required. Current First Aid Certification required or ability to obtain.
Minimum Requirements:
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet Carolina SeniorCare's auto insurance carrier's standards for coverage.
- CPR/First Aid certification required.
- LPN Licensure required.
Minimal Experience:
- Minimum of one year working with the frail or elderly.
- Two (2) years clerical or business experience, preferably in a health care environment is preferred.
- Experience with a personal computer, various software applications, email, facsimile, and copier. Intermediate to advanced computer skills.
