About This Position

Description:

Summary: This position provides direct support, including transportation to/from medical appointments, for people living with intellectual and developmental disabilities. The incumbent will act as the liaison between WCDC and medical professionals. The incumbent must be organized and have excellent written and oral communication skills.

Schedule: Monday - Friday, 9 a.m. - 1 p.m. Hours may vary due to transporting clients to/from appointments.

Rate of Pay: $19.93 per hour

Requirements:

Essential Duties and Responsibilities:

Includes the following and any other duties as assigned to meet the vision, fulfill the mission, and abide by the values of the organization.

  • Serves a liaison between WCDC and medical professionals providing care to the individuals.
  • Schedules and tracks all medical appointments for the individuals and determine and/or provides transportation.
  • Safely transports individuals to appointments.
  • Completes, gathers, and disseminates medical and other information as necessary to effectively communicate information between WCDC and medical professionals.
  • Administers medications, performs treatments, and implement nursing care plans.
  • Assists persons served with personal care issues such as toileting , feeding, hygiene, etc, as needed.
  • Interacts with consulting staff, family and the community in a positive manner. Supports WCDC's position during interactions with others. Operates according to the WCDC's mission and vision.
  • Attends person centered plan meetings as needed an provided medical guidance and input.
  • Develop strategies to help clients overcome their limitations and individualizes programs to provide for the needs of each person served.
  • Maintains medical files for residential individuals at the Center and in each residential home, making sure they are kept up to date and accurate.
  • Performs other duties as assigned.

Qualifications: Must be at least 21 years old, with a valid driver's license, and approved to transport clients. License may not be Provisionals and have a driving record that reflects responsibility and maturity. Complete all DDA Core Training successfully.

Education/Experience:

  • High School Diploma/GED
  • 3+ years' experience in the medical field
  • DDA experience and CNA preferred

Physical Demands/work Environment: Must be able to stand and walk 90% of work time; sit and/or drive 10%. Must be able to lift, carry, push, and/or pull 60 pounds; must be able to stoop, kneel, crouch and reach. All of the requirements are necessary in order to properly supervise and assist individuals with disabilities. Should be able to climb stairs/ladder and have good balance. Vision is required in order to drive.
