Description:

This is a multi-site role supporting patients across Salem, Peabody, and Gloucester.

Opportunity Overview

Under the supervision of the Director of Behavioral Health, the Behavioral Health Provider is a Masters or Doctoral level licensed clinician providing integrated primary care/behavioral health services at North Shore Community Health. Pre-licensure clinicians are also welcome to apply, dependent upon availability of supervisors to support licensure.

Essential Duties and Responsibilities:

  • Provide direct treatment to patients.
  • Provide consultation to primary care staff pertaining to the behavioral health needs of their patients.
  • Consult with primary care providers on an ongoing basis pertaining to the patient’s condition and the patient’s medical and behavioral health care plan and treatment goals
  • Provide treatment for individuals of all ages and their families as needed. Modalities used include: brief treatment; diagnostic assessments; crisis intervention; crisis prevention and management (homelessness, detox, reporting suspected child and/or elder abuse, tenant issues).
  • Provide case management; referral coordination, follow-up, and collaboration with area agencies; Develop and access community support services as needed.
  • Provide education/information/assistance in obtaining access to long-term behavioral health care and other area human service agencies.
  • Provide education on chronic disease management.
  • Develop self-management goals as directed by the patient care team.
  • Provide culturally competent support services for patients depending on individual needs and preferences.
  • Identify barriers to care, problem solving and identifying solutions both within the health center and with other agencies as needed; provide patient support and advocacy; provide informal supportive counseling and encouragement; provide direct assistance with completion of forms and making appointments; provide all necessary follow up with patients; and, coordinate and/or provide interpreting for patient appointments within and outside of NSCH.
  • Network/collaborate with community leaders, health care providers, community-based organizations, and others who contribute to the ongoing assessment of the health needs of this group of patients.
Requirements:

Required Skills and Abilities:

  • Experience in community health care.
  • Must have reliable transportation to travel between sites.
  • Demonstrated strong communication skills with individuals and/or groups
  • Spanish or Portuguese fluency preferred
  • Experience working with an electronic health record, experience with Epic strongly preferred. Literacy in Microsoft Office applications (e.g., Excel and Word)
  • Maintains strict adherence to the NSCH Confidentiality policy
  • Incorporates NSCH values, principles, and mission statement into daily activities
  • Complies with all NSCH personnel policies and procedures
  • Strong analytical, problem-solving, planning, and prioritization skills
  • Self-motivated and self-directed to work within a team and/or work independently
  • Maintain CEUs as required by the state.
  • Ability to complete NSCH required Compliance Training before the deadline.
  • Ability to work one evening shift per week (until 7 pm)

Education & Experience

  • LICSW, LMHC, LMFT, PhD, or PsyD strongly preferred.
  • If pre-licensed LMHC, PsyD, PhD, (or LCSW), offer is contingent upon availability of a supervising clinician. It is expected that the pre-licensed clinician provide proof of licensure within 6 months of completing supervision hours and being eligible for the licensing exam.
  • Four or more years of experience and training in a community health or mental health center setting; and/or related experience in health/human services and/or social services agency preferred but can mentor and train newer clinicians

  • Regular interaction with colleagues, supervisors, and/or clients occurs in person, by phone, and via digital communication platforms.
  • Standard office hours apply, with occasional flexibility required to meet deadlines, attend meetings, or respond to organizational needs.
  • Work may require sitting or standing for extended periods, using computers and other standard office equipment.
  • Employees are expected to follow all organizational policies and procedures, including those related to safety, confidentiality, and ethical conduct.

Physical Requirements

  • Occasional lifting of materials (e.g., boxes, equipment) weighing up to 50 pounds
  • Ability to sit or stand at a desk for extended periods, with consistent use of computers and telephones
  • May be required to travel to multiple sites within a single day

As a health center, NSCH ensures that all employees comply with the annual requirements for vaccinations and health screenings. All employees must be vaccinated against COVID-19 as a condition of their employment unless they have been granted a religious or medical exception.
