The LOGAN Nurse is responsible for overseeing and monitoring the health care services for LOGAN clients and implementation of nursing support plans for assigned caseloads. Provide first aid, basic medical treatment and basic medical evaluation for individuals served and staff at the location in emergent situations; and provide staff training as directed. As requested, the nurse will provide input into agency health, medical and safety related systems.

Requirements:

RESPONSIBLITIES

Essential Functions - General

  • Provides medical consultation and evaluation in emergent situations for individuals served.
  • Attends staff meetings.
  • Ensures compliance with licensing regulations, accreditation and agency policies and procedures.
  • Provides a variety of health, medical and safety related training to staff as directed.
  • Provides input as directed into agency policy, procedure and practice related to health, medical and safety issues.
  • Reviews all risk plans for effectiveness.
  • Teaches Core A and B for staff as assigned.
  • Administers medications to individuals served as directed.
  • Participates in On-Call rotation phone and responds to emergency medical issues as directed.
  • Participates on committees as assigned and performs related duties.
  • Performs other duties as assigned.

Essential Functions - Supported Living and Group Living

  • Provides consultation of all health/medical needs as they apply to each individual in service.
  • Provides consultation with staff with regards to medical care of individuals served.
  • Works cooperatively with Program Managers and Case Coordinators for risk management plans.
  • Oversees the completion of an initial annual nursing assessment of each individual and annually thereafter.
  • Ensures that an individual’s health is maintained at its optimal level and ensures that all changes in an individual’s health status are promptly addressed.
  • Documents client encounters and follow up in Accel Provide utilizing the medical note and vitals.
  • Communicates with parents and guardians as it relates to health and wellness of the clients.
  • Participates in client team meetings as requested by the team or Director of Nursing.
  • Reviews medical forms provided by the Leads and Program Managers and documents review.
  • Uploads medical documents into Accel Provide as needed.
  • Visits clients who are hospitalized the next business day following hospitalization or as directed by DON. Continue to visit as needed or requested until hospital discharge. Follow up with the client’s team following each visit.
  • Takes an active role in working with the team following discharge from the hospital and outpatient medical procedures.
  • Provides written reports, as requested, to the individual’s support team.
  • Maintains primary contact with pharmacy for residential client medication issues.
  • Trains staff on pharmacy procedures.
  • Assists staff with pharmacy issues as needed.
  • As directed, serves as an agent to agency psychiatrist in order to give verbal orders for prescribed medication.
  • Maintains on-going relationship with pharmacy which includes working with consulting pharmacist to set quarterly pharmacy reviews, coordinating the review and complete follow up tasks based on the identified issues and maintains pharmacy delivery receipts to verify and track medication.

Essential Functions - Wellness Coordination

  • Provides the development, maintenance and routine monitoring of the waiver participant’s Wellness Coordination Plan and the medical services required to manage the client’s health care needs.
  • Documents all Consultations/Reviews and Face-to-Face in Accel in accordance with the Tier requirements for individuals.
  • Maintains communication with all required parties for Wellness Coordination.
  • Develops and implements a Wellness Plan for individuals.
  • Collects data for Quarterly Assessments/Risk Assessment Tool updates in Accel.
  • Maintains all documentation within the required allotted time.
  • Attends Individual Support Plan meetings when required.

General Responsibilities

  • Assists in promoting the health and safety of individuals served by LOGAN, visitors, and other staff.
  • Serves as role model for individuals served and other staff.
  • Serves as an advocate and protects the human rights of all individuals served.
  • Participates in training as required by supervisor or for all LOGAN staff.
  • Facilitates professional growth by keeping current on new philosophy, research, technology, program strategies, etc.

QUALIFICATIONS

Education

  • High school diploma or equivalent required.
  • Licensed as a Licensed Practical Nurse in the State of Indiana.

Experience

  • Two years’ experience required. Experience may include a combination of life experience and working with individuals with developmental disabilities

Knowledge, Skills and Abilities

• A positive attitude to support individuals with disabilities.

• A willingness to learn and implement positive approaches to instruction and behavior management.

• Competent computer skills.

• Ability to effectively communicate orally and in writing.

• Ability to lift a minimum of 50 pounds.

• For those positions with driving responsibilities, you must have a valid driver's license, vehicle insurance, vehicle registration, and an acceptable driving record that meets the risk management standards for insurability on LOGAN's commercial auto insurance policy.
