The Resource Facilitator will work with a team of healthcare professionals managing complex catastrophic injuries, to assess, collaborate and provide support to injured individuals and their families by identifying and connecting them with essential resources to aid in their recovery, including housing, medical equipment, home care, food assistance, transportation, and other critical services that promote independence and well-being.
DUTIES AND RESPONSIBILITIES:
- Resource Coordination: research and maintain up to date information on community, state, and national resources that support individuals with disabilities.
- Patient Support: Work directly with patients and their families to assess needs and create tailored plans to access appropriate services
- Navigation Assistance: Guide patients/ families through application processes for social services, Medicaid/Medicare/SSD, housing, food programs, transportation services, medical equipment, and other support services.
- Advocacy: Advocate for patients to ensure they receive equitable access to community resources and services.
- Collaboration: Work closely with care management team, healthcare providers, social workers, and community partners to ensure a comprehensive support network.
- Documentation: Maintain accurate records of patient interactions, referrals, and outcomes in compliance with organizational policies and confidentiality regulations.
- Crisis Response: Provide immediate support and resource options during urgent or crisis situations.
- Education & Outreach: Educate patients and families on available programs and how to navigate complex service systems.
QUALIFICATION REQUIREMENTS:
- Education: Bachelor’s, or master’s degree, in social work, Human Services, Psychology, or a related field.
- Experience: 2+ years in case management, social services, or resource coordination, preferably working with individuals with disabilities.
- Familiarity with local, state, and national support programs.
- Skills:
- Strong research and problem-solving skills.
- Excellent interpersonal and communication abilities.
- Ability to manage multiple cases and prioritize effectively.
- Proficiency in Microsoft Office and case management software.
- Other Requirements:
- Knowledge of disability rights, accessibility standards, and community-based services.
- Ability to work independently and as part of a team.
- Sensitivity and respect for diverse backgrounds and individual needs.
- Preferred Qualifications:
- Bilingual or multilingual abilities.
- Experience with Medicaid, SSI/SSDI, housing assistance, and food security programs.
- Understanding of trauma-informed care principles.
