Essential Job Functions
- Provides individualized nursing care across the lifespan in an ambulatory setting using the professional nursing process, including focused assessment and clinical decision-making.
- Collects and interprets patient data to identify needs and support appropriate, patient-centered care planning.
- Delivers care across the continuum, including prevention, health maintenance, diagnosis, treatment, and follow-up.
- Coordinates care within the RN scope of practice, including triage, screenings, patient education, and collaboration with providers and the care team.
- With the appropriate training, may assist with providing comprehensive care coordination activities, including Chronic Care Management (CCM), Transitional Care Management (TCM), Principal Care Management (PCM), INR management and the American Diabetes Association - Certified Diabetes Education Program.
- Facilitates safe care transitions through effective communication with patients, families, and care team members.
- Promotes patient safety through adherence to established protocols and adhering to best practices and standard of care.
- Documents care accurately in the patient health record in accordance with organizational and regulatory standards.
- Provides patient and family education tailored to individual needs.
- Assists with clinical procedures as directed by providers.
- Maintains compliance with departmental policies, procedures, workflows, and regulatory requirements.
- Maintains professional knowledge, licensure, and required certifications.
- Performs other job duties as assigned to effectively meet the needs of the patients, the department, and the organization as a whole.

Requirements:Education
- Associate’s Degree (two year college or technical school) Required, Field of Study: Nursing
- Bachelor’s Degree (four year college) Preferred, Field of Study: Nursing
Experience
- 3-5 years Nursing experience in a clinic setting preferred
Computer Skills
- Proficient in basic computer skills
- EPIC knowledge and experience preferred
Certifications & Licenses
- BLS
- Registered Nurse License eligible to practice in the State of Wisconsin
