Registered Nurse (RN) – Admissions, Discharge & Transfer (ADT)
Location: Boone, North Carolina | Inpatient | Non-Exempt

Compensation: Starting at $34.00 per hour or more, based on education, experience, certification, and internal equity
Status: Non-Exempt, Full-time

What You’ll Do as an ADT RN Admissions, Discharge & Transfer Coordination- Conduct comprehensive admission assessments, including history, physical status, and record review.
- Develop and implement individualized discharge plans in collaboration with physicians, social workers, and care teams.
- Coordinate safe, timely patient transfers between units or facilities with complete documentation and communication.
- Ensure continuity of care across all transitions.
- Advocate for patients and families throughout transitions of care.
- Provide clear education on medications, discharge instructions, and self-care.
- Address concerns with empathy and professionalism to support a positive patient experience.
- Work closely with interdisciplinary teams to ensure seamless care coordination.
- Maintain clear, timely communication with providers, staff, patients, and families.
- Support care teams during high-volume or complex transition scenarios.
- Maintain accurate and timely documentation in compliance with regulatory standards.
- Participate in quality improvement initiatives related to admissions, discharges, and transfers.
- Promote patient safety and adherence to organizational policies and procedures.
- Assist in emergency situations as needed, following established protocols.
- Respond promptly to changes in patient condition.
- Graduate of an accredited nursing program
- Associate Degree in Nursing (ADN) required
- Minimum of one (1) year of Registered Nurse experience required
- Strong communication, organizational, and critical thinking skills
- Ability to work effectively in a fast-paced clinical environment
- Current Registered Nurse (RN) license in the State of North Carolina
- Basic Life Support (BLS) from the American Heart Association required at time of hire
- Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and PEDS training required within one (1) year of hire
- NIH Stroke Scale (NIHSS) certification required within one (1) year of hire
- Strong interpersonal and communication skills
- Ability to deliver safe, effective patient care within regulatory and organizational guidelines
- Understanding of care transitions and the continuum of nursing care
- Strong organizational and multitasking abilities
- Commitment to exceptional customer service and patient-centered care
- Ability to lift, position, push, pull, and transfer patients or equipment weighing 50+ pounds
- Prolonged standing, walking, bending, squatting, and twisting
- Manual dexterity for clinical tasks and documentation
- Near visual acuity for detailed patient assessments
- Exposure to bloodborne pathogens and infectious materials
- Frequent exposure to medical equipment, chemicals, and environmental hazards
- Requires flexibility for days, evenings, nights, weekends, holidays, and emergency situations
- Extended standing and walking during shifts (8+ hours)
- Occasional exposure to radiation, vibration, or confined spaces
