Job Description

Behavioral Health Discharge Planner- Social Worker

  • POSITION SUMMARY: Assesses plans and coordinates the discharge planning process for high risk behavioral health patients having complex discharge planning needs. Ensures timely discharges through early intervention and identification of post emergency room /post-hospital care needs. Monitors, and follows through as needed, to ensure that continuity of care needs is met. Maintain relationships with DSS and placements agencies assuring direct access to next level of care. Provides, as needed, discharge planning and placement support in Emergency Department, inpatient psychiatric unit and outpatient behavioral health services at Southeastern Psychiatry Clinic. Drives three outcomes starting in ED with outpatient, Inpatient admission, or Transfer to another inpatient or placement program.

Works with Ombudsmen to assure patients are not inappropriately left in the Emergency Department. (Federal and State Anti Dumping Regulations).

EDUCATION, CREDENTIALS, TRAINING and EXPERIENCE:

Preferred Requirements: Bachelor’s Degree in Social Work, Sociology or Psychology from an accredited institution. Working knowledge and understanding of the social work and discharge planning processes gained through previous experience or an approved internship. Minimum of 1 years’ experience as medical social worker.

Required: Master’s degree in Social Work, Sociology or Psychology from an accredited institution. Plus a minimum of one years’ experience working

as a Discharge Planner/Patient Placement Worker in a Health Care Setting.

ADDITIONAL SPECIALIZED KNOWLEDGE/SKILLS: Incumbent must demonstrate an ability to apply age-appropriate principles of communication and social work practice relevant to neonatal, infant, pediatric, adolescent, adult and geriatric patients. Incumbent must demonstrate competence in the use of equipment and application of procedures before being allowed to perform independently.
