About the Role:

The MDS Coordinator plays a critical role in ensuring the accuracy and compliance of Minimum Data Set (MDS) assessments within long-term care facilities. This position is responsible for coordinating and overseeing the comprehensive assessment process that informs care planning and reimbursement. The MDS Coordinator collaborates closely with interdisciplinary teams to collect, analyze, and report resident data, ensuring regulatory standards are met and quality care is delivered. This role requires meticulous attention to detail and a deep understanding of federal and state regulations related to MDS documentation. Ultimately, the MDS Coordinator contributes to optimizing resident outcomes and facility performance through effective data management and communication.

Minimum Qualifications:

  • Minimum of 2 years MDS experience.
  • Minimum of 2 years experience in long-term care or skilled nursing facility settings.
  • Current Licensed Practical Nurse (LPN) license in the United States.
  • Demonstrated knowledge of MDS 3.0 assessment processes and federal/state regulations.
  • Proficiency with electronic health record (EHR) systems and MDS software applications.
  • Strong organizational and communication skills.

Preferred Qualifications:

  • Current Registered Nurse (RN) license in the United States.
  • Certification as a Certified MDS Coordinator (CMS or other recognized credential).
  • Experience with quality improvement initiatives and data analysis in a healthcare setting.
  • Familiarity with Medicare and Medicaid reimbursement processes.
  • Advanced training in gerontology or long-term care management.
  • Experience working with interdisciplinary care teams.

Responsibilities:

  • Conduct and coordinate comprehensive MDS assessments for residents in accordance with federal and state guidelines.
  • Collaborate with nursing, therapy, and other clinical staff to gather accurate resident information and ensure timely completion of assessments.
  • Review and validate MDS data for accuracy and completeness prior to submission to regulatory agencies.
  • Maintain up-to-date knowledge of MDS regulations, coding guidelines, and reimbursement methodologies.
  • Provide training and support to clinical staff on MDS processes and documentation requirements.
  • Monitor and track assessment schedules to ensure compliance with regulatory deadlines.
  • Assist in quality assurance initiatives by analyzing MDS data trends and identifying areas for improvement.
  • Serve as a liaison between the facility and external agencies during audits and surveys related to MDS documentation.

Skills:

The MDS Coordinator utilizes clinical knowledge and regulatory expertise daily to accurately complete resident assessments that impact care planning and reimbursement. Strong communication skills are essential for collaborating with nursing staff, therapists, and administrative personnel to gather and verify resident data. Analytical skills are applied to review assessment data for accuracy and to identify trends that inform quality improvement efforts.
