RN Home Health PT at Carteret Health Care – Morehead City, North Carolina

Responsible to Case Manager as appropriate & Team Leaders and Clinical Manager. Provides patient care as directed by Case Managers. Consults with and keeps Case Managers informed of activities, needs, incidents, complaints or problems related to delivery of quality patient care.

The continuous improvement in assessing, developing, and delivering a well-organized plan of care as it relates to nursing services and case management.

HHA supervision, patient care, monitoring, and communication with other appropriate team members and clinical manager.

Working within the scope of the Nurse Practice Act regarding judgment, nursing skills, education, and experience.

The following only applies to the "On Call" nurse position: This position covers on call hours from 4:30 PM through 8:00 AM, while the office is closed. Additionally, this position may pick up additional visits in the late afternoon as needed.
