Position Overview

The Choice Care Coordinator (LPN/LVN) supports veterans by providing patient-centered care coordination, education, and follow-up services. This role functions within the Patient Aligned Care Team (PACT) framework and/or supports veterans not aligned to a PACT model, coordinating VA Choice/Community Care services in accordance with VA policy. The position collaborates closely with providers, clinic leadership, and VA partners to ensure timely access to care, continuity of services, and a positive veteran experience. All duties are performed within the scope of Licensed Practical Nurse practice

Core Responsibilities

Veteran & Customer Experience:

· Provide excellent customer service to veterans, colleagues, and clinic visitors, both in person and over the phone.

· Establish and maintain a warm, welcoming, and professional environment.

· Collaborate with clinic leadership to address and resolve veteran concerns or complaints in a timely and respectful manner.

Choice Care Coordination & Retention:

· Serve as a point of contact for veterans, clinic staff, vendors, and VA partners regarding Choice/Community Care processes and guidelines.

· Provide patient, vendor, and provider education related to Choice/Community Care consults, timelines, and documentation requirements.

· Document all Choice/Community Care–related interactions in CPRS in accordance with VA standards.

· Assist with initiating, tracking, and closing out Choice/Community Care consults and appointments under provider direction.

· Maintain tracking tools and perform outreach to support timely scheduling and follow-up for veterans awaiting Choice/Community Care services.

· Obtain, route, and ensure external medical records related to Choice/Community Care are appropriately scanned and indexed in CPRS.

· Coordinate with primary care providers and the VA Integrated Care/Community Care department to support follow-up care and resolution of administrative issues.

· Verify and update veteran demographic information in Vista and other VA systems as required.

· Support communication with Choice/Community Care vendors to facilitate provider-directed orders, imaging, and prescription coordination.

· Utilize approved CPRS templates and documentation tools to ensure consistent and compliant recordkeeping.

· Support patient enrollment, appointment scheduling, follow-up visits, and nurse visits.

· Promote continuity of care by reinforcing follow-up plans and care instructions as directed by the provider.

· Support “Save & Recapture” initiatives to maintain patient engagement and access to care.

Enrollment Growth & Outreach

· Participate in clinic outreach and enrollment events to support growth initiatives.

· Educate veterans on available services and care options as appropriate.

Strategic & Operational Support

· Embrace and support new clinical and operational initiatives.

· Assist with workflow improvements and clinic priorities as directed by leadership

Compliance

  • Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items.

Clinical Outcomes

  • Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.

Customer Service

  • Must provide excellent customer service to each veteran, both in person and over the phone, as well as to fellow colleagues and clinic visitors.
  • Establish and maintain a warm, welcoming and professional atmosphere for our colleagues and veterans, ensuring needs and expectations are consistently met.
  • Collaborate with Clinic leadership to resolve veteran concerns or complaints.

Enrollment Growth

  • Participate in the clinic’s outreach events to help support the clinic’s enrollment initiatives.
  • Save & Recapture: Support patient enrollment and retention in appointments, follow-up visits or nurse visits.
Requirements:
  • Diploma or certificate of an accredited program of practical or vocational nursing as approved by the state.
  • Minimum one-year experience as an LPN/LVN in a related primary care or ambulatory care setting; government healthcare environment preferred. Specific requirements could vary based on individual VA contract.
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.
  • Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – e.g., AHA valid for two years) and in ACLS as specified by individual VAMC contract.
  • Proficiency in written and spoken English.
  • Strong computer skills, including EMR experience.

  • This is considered a safety sensitive position.

Final compensation will be based on relevant factors including work experience, skills, certifications, and internal equity.
