Location: On-site (Hipaa compliant office)
Schedule: MondayFriday
First person: Monday, Tuesday, Thursday, Friday and Saturday 11am - 8pm EST // Sunday and Wednesday OFF
Second person: Monday, Tuesday, Wednesday, Friday and Sunday 10AM - 7PM EST // Saturday and Thursday OFF
Job Profile Summary
Our clients provide high-touch care navigation services for dementia patients and their caregivers using an AI-first platform. The Care Coordinator onboards caregivers and patients to the client's platform, maintains regular check-ins via in-app chat or recorded calls (and occasional in-person meetings when appropriate), triages concerns, and coordinates solutions with clinical teams and community partners. The goal is to reduce caregiver burden by continuously understanding needs and ensuring the platform effectively meets them.
Key Responsibilities
Onboarding & Relationship Building: * Onboard caregivers and dementia patients onto the Client's platform and ensure profiles are kept up to date.
Establish a regular check-in cadence; be available in person (as needed), virtually, or by phone/in-app chat (calls/chats recorded per policy).
Build trusting, longitudinal relationships with patients and care partners.
Navigation & Coordination: * Understand the needs and concerns of dementia patients/caregivers; provide empathetic, patient-centered support.
Serve as the single point of contact for care needs; advocate for timely resolutions.
Connect patients to community resources and facilitate completion of clinical referrals using effective communication skills.
Conduct targeted outreach to high-risk dementia patients/caregivers to ensure timely, efficient delivery of care.
Platform & Data Use * Learn and use current/new features across our clients AI platform to support caregivers (note-taking, triage tools, research/collation assistants such as ChatGPT, Gemini, etc.).
Assist with our client's in-house system setup, connectivity, and troubleshooting.
Receive and analyze digital inputs from the client's in-home platform and align data feeds to clinical response protocols.
Documentation, Quality & Compliance * Document exclusively within the client's platform (no external notebooks) to ensure all notes are secure and encrypted (HIPAA compliance).
Support quality and process-improvement by documenting issues, submitting internal tickets, and contributing validation feedback to enhance platforms and workflows.
Required Qualifications
Education * Nursing or Social Work educational background.
Experience * Caregiving background required, ywith experience dealing with dementia patients
Must have served US population and possess working knowledge of the US healthcare system and insurance.
Care Coordinator or Population Health Navigator experience preferred.
Minimum 3 years working with patients in a clinic or customer-service setting, or equivalent experience collaborating with patients, clinicians.
Proficient in English (current focus on US market).
Spanish may be needed as the program scales.

Understanding of the physical, social, and psychological needs of dementia patients and caregivers.
Knowledge of medical terminology preferred.
Ability to maintain composure in stressful situations.
Technical & Compliance Skills * Comfortable using an AI-based platform and SAS infrastructure at a basic, non-coding level (e.g., navigation, note-taking, analytics views).
Proficient with research and collation tools (e.g., ChatGPT, Gemini) to surface relevant resources for caregivers.
Working knowledge of HIPAA regulations and adherence to strict data-handling rules:
All documentation must be within the client's platform; no external notebooks.
All stored information is encrypted per policy.
Basic computer skills; accurate, timely digital note-taking
