Description:

The Clinical Case Manager – The Lodge provides client-centered, recovery-oriented clinical case management services within The Lodge, a six-bed, quasi-independent residential living program for adults with serious mental health needs who are preparing for greater independence. While residents demonstrate readiness for semi-independent living, they continue to require structured programming, clinical oversight, and coordinated case management services to support stability, skill development, and community integration.

This position is responsible for assessment, service planning, coordination of care, therapeutic interventions, and ongoing monitoring of residents’ progress while promoting autonomy, safety, wellness, and recovery. The Clinical Case Manager works collaboratively with residents, residential staff, outpatient providers, community partners, and internal agency teams to ensure continuity of care and compliance with regulatory, clinical, and safety standards.

Requirements:

MINIMUM QUALIFICATIONS

  • Master’s degree in psychology, social work, counseling, psychosocial rehabilitation, or a related human services field.
  • Eligibility for New Hampshire clinical licensure (LCMHC, LICSW, or equivalent) and third-party reimbursement.
  • Experience providing community-based mental health, residential, or case management services preferred.
  • Relevant life experience may be considered in lieu of direct professional experience, as appropriate.
  • Ability to work independently and collaboratively within a multidisciplinary team.

Additional Preferred Qualifications

  • Prior experience working in residential or supportive housing programs.
  • Experience supporting individuals transitioning toward independent living.
  • Familiarity with New Hampshire CMHC regulations and documentation standards.
  • Experience facilitating groups focused on life skills, recovery, or wellness.

OTHER EXPECTATIONS

  • Maintain a valid driver’s license, acceptable driving record, proof of auto insurance, and reliable transportation.
  • Transport residents in a personal vehicle when required and appropriate, in accordance with agency policy.
