About the Role:

The Registered Nurse (RN) in a Skilled Nursing Facility (SNF) plays a critical role in delivering high-quality, compassionate care to residents requiring short-term rehabilitation or long-term nursing care. This position involves comprehensive assessment, planning, implementation, and evaluation of individualized care plans to promote optimal health outcomes. The RN collaborates closely with interdisciplinary teams, including physicians, therapists, and social workers, to ensure coordinated and effective care delivery. Additionally, the RN advocates for residents' rights and well-being while maintaining compliance with regulatory standards and facility policies. Ultimately, this role is essential in enhancing the quality of life for residents through expert clinical care and empathetic support.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of practice within the United States.
  • Graduation from an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Basic Life Support (BLS) certification.
  • Minimum of one year of clinical nursing experience, preferably in a long-term care or skilled nursing environment.
  • Strong knowledge of nursing principles, medication administration, and patient care standards.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) degree.
  • Experience working in a Skilled Nursing Facility or long-term care setting.
  • Advanced certifications such as Certified Rehabilitation Registered Nurse (CRRN) or Geriatric Nursing Certification.
  • Familiarity with electronic health record (EHR) systems and documentation software.
  • Demonstrated leadership or preceptorship experience.

Responsibilities:

  • Conduct thorough nursing assessments and develop individualized care plans based on residents' medical histories and current health status.
  • Administer medications and treatments accurately while monitoring for adverse reactions and effectiveness.
  • Coordinate with interdisciplinary team members to implement and adjust care plans as needed to meet residents' evolving needs.
  • Provide education and support to residents and their families regarding health conditions, treatments, and post-discharge care.
  • Ensure compliance with all federal, state, and local regulations, as well as facility policies and procedures.
  • Document all nursing care activities, observations, and resident responses accurately and timely in the medical record.
  • Respond promptly to emergencies and changes in residents' conditions, initiating appropriate interventions.
  • Participate in quality improvement initiatives and staff education to promote best practices in nursing care.
