Job Opportunity: Service Coordinator
Position Summary The Service Coordinator supports individuals with intellectual and developmental disabilities by planning, coordinating, and monitoring services that help them achieve their goals. Working under the Program Manager, this role uses a person-centered approach to ensure each individual receives high-quality, compliant, and meaningful support.
Key Responsibilities
1. Service Coordination
- Lead and document the person-centered planning process.
- Develop, implement, and manage each Individual Service Agreement (ISA).
- Complete all required paperwork accurately and on time.
- Help individuals identify strengths, needs, and personal goals.
- Maintain regular communication and interaction with assigned clients.
- Review ISA expectations, objectives, and data tracking for quality and compliance.
- Coordinate care with providers such as doctors, therapists, specialists, and psychologists.
- Conduct regular face-to-face visits in homes and the community (minimum one home visit per month).
- Ensure all services follow state regulations.
2. Program Coordination & Development
- Assess needs and recommend appropriate services.
- Schedule and monitor all waiver services to ensure delivery.
- Work with clinical team members to create individual support plans.
- Make sure goals, objectives, and documentation systems are timely and accurate.
- Act as a liaison between the agency and external service providers.
- Evaluate the effectiveness of services and communicate updates or concerns.
3. Client Advocacy
- Share important information with staff, agency programs, and outside human service agencies.
- Refer individuals to needed services and maintain follow-up contact.
- Help develop and secure essential support resources.
- Advocate for each individual and monitor the quality of services they receive.
4. Administrative & Networking Responsibilities
- Respond promptly to information and documentation requests.
- Promote a positive image of the agency and its services.
- Represent UCS professionally with the public and referral sources.
- Serve as an agency representative when assigned.
Qualifications
Education & Experience
Required:
- High School Diploma or equivalent
- Relevant Service Coordination experience or
- At least one year of experience in Developmental Services
Preferred:
- Bachelor’s degree in Human Services or a related field
Skills
- Proficiency using Microsoft Outlook, Excel, and Word
- Strong interpersonal and communication skills (written and verbal)
- Ability to manage detailed/sensitive documentation with integrity and meet deadlines
Licenses/Certifications & Other
- ·Valid Driver’s License
- Reliable transportation to transport clients or attend meetings
