Social Worker at Medical Center Hospital – Odessa, Texas

The Social Worker facilitates integerated and comprehensive patient services during hospitalization. The Social Worker provides ongoing assistance and consultation to the healthcare team through a comprehensive assessment of patient needs, planning and implementation of an interprofessional care plan, and communication of individual patient needs.

A. Responsible to: Director of Care Management

B. Workers Supervised: None

C. Interrelationships:

Works in partnership with Care Management

Team. Works in a close and collaborative role with physicians and health care professionals who are providing care and service. Works closely with patients and their families. Has frequent contact with office staff of physicians, plus other community providers.

Qualifications:

A. Education:

Bachelor’ degree in Social Work required; Texas license preferred.

B. Training and Experience:

A minimum of one-year experience in acute care setting preferred. Excellent verbal and written skills well rounded critical thinking skills, self-motivated, flexible, and goal oriented. Ability to tolerate frequent interruptions and demanding workload.

C. Job Knowledge:

Demonstrates knowledge of the principles of growth and development over the life span and possesses ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs
