What is the work?
· One-on-one case management including helping to resolve crises, assessing needs, connecting to resources, developing and implementing personalized plans to increase stability and achieve individual goals. Work with clients to assess strengths and barriers. Provide ongoing support through advocacy, counseling and problem-solving assistance in support of housing retention and stability goals
· Maintain regular communication with property management to collaboratively address client and community needs, including resolving conflicts, discussing account balances, and planning upcoming events and initiatives
· Collaborate with other service providers and co-workers to ensure services are delivered, and client concerns are addressed in a positive way that supports Housing First principles
· Partner in the Housing Resource Navigation team to support clients who are facing housing barriers including housing search, housing stability, and advocacy

· BSW or BA/BS in related field
· Experience with the Housing Support program, as well as knowledge of other housing subsidy programs and services
· Experience working in adult mental health or case management with adults with serious and persistent mental health diagnoses
· Thorough understanding of the case management process
· Experience working in a Housing First setting and/or an understanding of Housing First as a service approach
· High level of computer proficiency, strong communication and problem-solving skills, and the ability to maintain professional boundaries
· Commitment to equity and inclusion and ability to excel in a diverse environment
· Capable of relating to and advocating for diverse client needs and interests
· Valid driver’s license and clean driving record
