POSITION SUMMARY
-
The Infusion Center Pharmacist provides pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost-efficient medication therapy. Works with a multidisciplinary team exercising sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner. The pharmacist will oversee the operations of the pharmacy ensuring adherence to productivity and quality standards, internal policy requirements and external regulations.The Infusion Pharmacist is responsible for the optimization of drug therapy by identifying, resolving, and preventing drug-related problems and adverse events. The pharmacist accepts responsibility, authority, and accountability for management and provision of care in accordance with the current EAMC policies and procedures. The pharmacist is responsible for leading efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy and works to continuously improve process and practice in the department to enhance safety of medication therapy. Pharmacists must maintain competence in all aspects of current clean room operations, aseptic techniques, and pharmacy standards including the safe handling of hazardous medications (USP <795>, <797>, <800>).

POSITION QUALIFICATIONS

Minimum Education
-
Graduate of accredited school of pharmacy

Minimum Experience-
Have completed one of the following:
-
- An ASHP-accredited PGY1 residency program followed by a minimum of one year of pharmacy practice experience in the area precepted
- An ASHP-accredited PGY1 residency program followed by an ASHP accredited PGY2 residency and a minimum of six months of pharmacy practice experience in the area precepted
- Have three or more years of pharmacy practice experience in the area precepted if they have not completed an ASHP-accredited residency program

Required Registration/License/Certification-
Active and clear Registered Pharmacist license and Controlled Substance Permit from the Alabama State Board of Pharmacy

BLS and ACLS within 90 DOH/Transfer

Preferred Education-
PharmD and PGY-1/ PGY-2 Residency Completion
-
-
Preferred Experience-
3-5 years of inpatient hospital pharmacy practice
-
-
Preferred Registration/License/Certification-
Board of Pharmacy Specialties Certification or other applicable certification

Other Requirements-
N/A
