We are seeking a highly motivated Patient Outreach Coordinator to support our clinical team by engaging patients and ensuring they receive timely care. Key Responsibilities
- Conduct outbound patient outreach calls to schedule appointments, including:
- Annual visits
- Transitional care management
- Follow-ups
- Preventive screenings
- Assist patients with scheduling, rescheduling, and appointment reminders
- Support population health initiatives by helping patients complete recommended care
- Document patient interactions accurately in the electronic health record (EHR)
- Collaborate with providers and clinic staff to optimize scheduling and patient access
- Maintain professionalism and protect patient confidentiality (HIPAA compliance)
- Improve patient engagement and appointment completion rates
Compensation

Requirements:- 1+ year experience in: Medical office, patient scheduling, healthcare outreach, or call center environment
- Strong phone communication and patient service skills
- Experience with EHR systems (eClinicalWorks preferred)
- Ability to manage high call volumes while maintaining quality interactions
- Strong organizational and time management skills
- Bilingual candidates (Spanish, Vietnamese, or other languages) strongly encouraged to apply
