Physical Therapy Assistant (Home Health) at Vanura Homehealth Services Inc – Rancho Cucamonga, California

Currently licensed by the California Board of Physical Therapy
Minimum of one year in the acute care setting. Preferably with 2 years Home Health experience
Must be able to maintain strict confidentiality with regard to protected and sensitive information.
Must be able to solve problems independently and confidently by applying analytical and logical thinking.
Good sense and practice of proper body mechanics.
Responsible for providing initial and ongoing assessment to determine level of functioning, including OASIS Assessment at appropriate time points.
Responsible in gathering background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient's physical and psychosocial functioning.
Responsible in developing and revising the plan of care in consultation with the physician and other care-team members
Responsible in preparing clinical progress notes.

Preferably willing to cover the High Desert area - Adelanto, Palmdale, Barstow, Victorville, Hesperia, and Apple Valley.
