About This Position
ACCORD CARE is trusted in the provision of person-centered disability support services to NDIS participants. We provide Supported Independent Living, In-home care, Community Participation, Short-Term Accommodation and Community Nursing.
About the role
We are looking for team members in our supported independent living arrangements in suburb of Redbank Plains to provide Supported Independent Living and Community Access.
The support worker's role and responsibilities will include:
- Supporting participants with personal care and day-to-day activities including meal preparation, household tasks, and medication reminders.
- Supporting participants to access community, attend appointments, participate in social and recreational activities.
- Following individualized Behaviour Support Plans and Risk Assessments to ensure safe and effective care.
- Completing appropriate documentation through shift notes, incident reporting and data collection.
- Contribute to the team’s overall objective of providing the best outcomes for our clients.
- Work with your team leader to implement and review plans around daily service provision.
- Maintain a professional relationship with clients and adhere to the NDIS and company code of conduct.
- Above all, be responsive, reliable and accountable to our clients and your team

Qualification/Experience:
- Minimum Certificate III in Individual Support/Aged Care or Higher (or the ability to acquire an appropriate qualification)
- Experience supporting participants with complex diabetes.
- Competent in monitoring BGLs and recording results.
- Able to administer insulin (pen or syringe) safely.
- Experience testing and recording ketone levels.
- Able to follow diabetes care plans and protocols.
- Mental Health Experience
- Challenging Behavior Experience
