Position Summary

The Patient Navigator supports individuals and families in accessing healthcare and supportive services through outreach, education, and care coordination. This front-line role promotes awareness of low- or no-cost health programs, assists with public insurance enrollment, and helps remove barriers to care in a respectful, compassionate, and non-judgmental manner. The Patient Navigator works closely with clinical teams, community partners, and health plans to ensure high-quality patient experience.

Key Responsibilities

  • Conduct outreach and assist patients with eligibility screening, enrollment, and referrals.
  • Support care coordination, including DME orders and specialist appointment follow-up. Provide health education and case management support to diverse populations.
  • Assist with enabling services such as HIV/STI testing, mobile outreach, and community events.
  • Communicate with health plans, providers, and patients to schedule appointments and close gaps in care.
  • Answer inbound calls and accurately document patient interactions.
  • Maintain data integrity and support quality improvement initiatives.
  • Collaborate with clinical teams to deliver excellent patient-centered care.
  • Travel between clinic sites as needed.

Qualifications

  • High School Diploma or GED required; college degree preferred
  • HIV 501 Certification required
  • Knowledge of healthcare systems, social services, and community resources
  • Strong organizational, communication, and interpersonal skills
  • Ability to work effectively with diverse populations
  • Bilingual English/Spanish preferred

  • Level 2 background clearance and required immunizations (MMR, Hep B, Varicella, Tdap, PPD).

Requirements

  • Level 2 background clearance required
  • Proof of immunizations for (1) Tdap, (2) MMR’s, HEP B vaccination, and either a history of chickenpox (varicella), or documentation of the varicella vaccine, a PPD Test is done once upon hire.
