About This Position

Job Summary |
- This position reports to the Nursing Supervisor, Home Care, and is responsible for both providing skilled nursing services to patients in their homes and performing remote monitoring duties. The role involves assessing patients, developing individualized care plans, educating patients and their families, and conducting daily reviews of patient clinical data and symptoms for those in the remote monitoring program. The nurse will also handle telephone and/or video calls to assess for clinical changes, provide disease management education, and report urgent findings to the appropriate healthcare team to help minimize hospital re-admissions and emergency department visits.

Minimum Qualifications |
- Education
- Bachelor’s degree in nursing preferred
- Experience
- Two years of home care nursing experience preferred. Will also consider medical/surgical experience.
- Prefer at least 3 years of clinical experience in cardiac and respiratory care
- Licensure and/or Certification
- Current and valid licensure to practice Nursing in the Commonwealth of Massachusetts required.
- BLS healthcare provider from American Heart Association or American Red Cross required within 30 days of start.
- Skills
- Excellent customer service skills.
- Must be able to read, write, and communicate in English.
- Computer skills required.
- Ability to maintain confidential information.
