About This Position

Clinical Navigator

Job Summary: Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within WMCHealth and the community. Facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians.

Responsibilities:

  • Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
  • Serve as an essential link between patients and all other care providers. Facilitate appointments for consultants and support services within established service standards, including social workers, dietitians, physicians, etc.
  • Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
  • Develop patient education programs and tools.
  • Complete new patient intake. Including, but not limited to: collect outside medical records, organizing medical records for physician/provider review, registering patients in EMR(as required), and scheduling initial consultations and multidisciplinary clinic visits (when appropriate)
  • Complete all post-clinic follow-up (referrals to internal and external organizations, ordering of DME etc.)
  • Manage the center’s telephone, fax, e-mail, and triaging systems (where and when appropriate)
  • Complete authorizations for medications and DME when necessary.
  • Digitize or transcribe outside documentation into EMR, as required.
  • Schedule and facilitate internal and external Center meetings. External meetings frequently occur with institutional departments/services, State/Federal agencies, Not-for-Profit agencies, vendors, and various professional partners.
  • Manage onboarding, ongoing training, and professional development of administrative and clinic staff members.
  • Contribute to performance reviews or performance improvement plans for administrative clinic staff members.
  • Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources.
  • Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes.
  • Deliver monthly and quarterly report card to committees and other groups which demonstrate outcomes and performance improvement activities.
  • Evaluate Radiologist report for appropriate birads codes; evaluates and tracks recommendations. Adhere to established productivity benchmarks, if appropriate and needed
  • Adhere to budgeted operational expenses associated with the service.
  • Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff, etc.).
  • Other tasks as assigned

Qualifications/Requirements:

Experience:

  • 1-3 years of experience in a clinical healthcare setting, required

Education:

  • BSN or Bachelor’s degree in Social Work or related field, required

Licenses / Certifications:

  • Valid NYS license as a Registered Nurse or Social Worker (LCSW, LMSW, or LMHC), required
  • BLS certification, required
  • OCN certification or equivalent, preferred

Other:

  • Requires excellent leadership, organizational, written, and verbal communication, and excellent interpersonal skills.
  • Requires problem-solving, decision-making, and critical thinking.
  • Must be able to work in a self-directed environment, with an ability to work with and lead a team.
  • Excellent presentation skills
