About This Position

And we know for sure that to achieve success, this road must be traveled with as much support as possible.


Care Manager is responsible for advocating for and assisting patients with every aspect of their physical and mental health by collaborating with various medical providers and internal team members.

Essential Duties and Responsibilities include the following and other duties that are assigned:

  • Learns appropriate assessment practices and techniques to obtain necessary information from clients, family and/or involved collaterals for the identification of service needs.
  • Creates and maintains partnerships within the community to be used for patient-centered care.
  • Receives training and assists in the coordination and provision of access to available alcohol and other drug and ancillary services, such as housing, childcare, medical and financial services.
  • Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
  • Identifies patient care requirements by establishing personal rapport with patients and other persons in a position to understand care requirements.
  • Participates in monitoring client treatment progress and/or the performance of utilization review activity.
  • Monitors status of patients with referral follow-ups.
  • Documents patient care services by charting in patient records.
  • Assures quality of care by adhering to Center of Excellence (COE) standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following standards of care and governing agency regulations.
  • Maintains data for quality improvement and quality control efforts; produces reports as needed for COE audits.
  • Maintains continuity among staff with morning meetings and follow-ups.
  • Maintains patient information confidentiality and privacy.
  • Assists in the orientation of clients to the program, policy, procedures, and treatment process.
  • Learns to develop and implement a client service plan based on the results obtained from the assessment process.
  • Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
  • Performs other related duties as required.

REQUIRED KNOWLEDGES, SKILLS, AND ABILITIES:

  • Knowledge of social case work principles and methods.
  • Knowledge of addiction as a disease and its treatment.
  • Knowledge of current social, economic and health problems.
  • Ability to establish and maintain an effective working relationship with clients from various cultures and social economic backgrounds.
  • Ability to communicate effectively, both orally and in writing.

MINIMUM EXPERIENCE AND TRAINING NECESSARY:

A bachelor’s degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice, or other related social sciences with personal recovery experience

OR
An Associates degree in a social science field with any equivalent combination of experience and training with lived recovery experience.

Ability to interpret regulations, policy and procedures and apply them accordingly.

Ability to work with persons with physical, mental, or emotional disabilities or who are economically disadvantaged or who are involved with the criminal justice system.

Ability to establish and maintain effective working relationships with other members of staff, outside agencies and institutions and the public.

Pay: $18-$21 per hour
