RN CASE MANAGER

Team: ICHC

Salary Grade: Grade 20$90,369 - $101,711 DOE

Reports To: Nursing Manager

Schedule: 40 hours per week

FLSA Status: Salaried & Not Eligible

POSITION SUMMARY

The Ilanka Community Health Center RN Case Manager works collaboratively with all members of the medical and behavioral health team to ensure access to appropriate, integrated care for the patients and clients of ICHC.

ESSENTIAL DUTIES

· Supervises Referral Coordinator position and oversees referral operations.

· Analyzes data to identify high risk and high-need patient populations for targeted case management services.

· Facilitate treatment plan changes, in consultation with medical providers, for patients requiring more intensive or specialized healthcare services.

· Assists patients in accessing necessary care resources, including the procurement of durable medical equipment and medications, particularly when resources are limited.

· Responsible for the continuity of patient care throughout the PCMH environment; serving as the liaison between Behavioral Health and Clinical departments, including Medications for Assisted Treatment (MAT) program.

· Coordinates and facilitates case management coordination meetings.

· Applies for and manages all aspects of Special Diabetes Program for Indians and any other specialized care management funding opportunities.

· Coordinate Narcan education, distribution and community outreach.

· Assists with patient triaging.

· As needed, provide direct patient care as registered nurse which may include Saturday coverage.

· Maintain competencies and participate in CLIA waived testing and associated activities as needed.

· Collaborate with the appropriate Native Village of Eyak departments in response to Tribal Member Social Determinants of Health needs.

· Serves as initial point of contact for patient Video teleconferences.

· Participates in Quality Assurance, Quality Improvement and Risk Management program.

· Maintain a high level of confidentiality in accordance with HIPAA and HITECH regulations, which includes only accessing appropriate information needed to perform job duties.

ESSENTIAL DUTIES OF ALL STAFF

Able to assess patient’s knowledge base and learning style, building rapport and providing education in a calm and caring manner.Maintains active listening skills and motivational interviewing techniques.Adept at providing clear verbal and written communication with patients, families and care teams.Accurately uses Electronic Medical Record for necessary documentation.

ABILITIES

Able to identify risks, unmet needs and barriers to care; prioritizing interventions based on urgency and risk.Ability to balance direct services, coordination, and administrative responsibilities.Ability to apply clinical judgment and critical thinking to address complex patient situations and barriers to care.Proficient in managing multiple cases simultaneously, prioritizing workload effectively.Ability to adapt to changing patient needs, priorities, and healthcare environments, including evolving clinical and organizational requirements.Able to respond to emergency situations within clinic as well as participate in clinic response when assisting local hospital or city Incident Management Team in an emergency event.Able to attend trainings.

EDUCATION AND EXPERIENCE

EDUCATIONRequired: Graduate of an accredited school of nursing.

WORK EXPERIENCERequired: : Minimum of three (3) years of RN experience in a clinical setting.Preferred: : Minimum of one (1) year of case management experience.

: Minimum of one (1) year supervisory experience preferred.

CERTIFICATES / CERTIFICATIONS / LICENSESRequired: Basic Life Support (BLS) or CPR Pro Certification required. : Case Manager certification. Or willing and able to achieve Case Manager certification within three (3)years of hire.
