About This Position
Advocates for and provides services to individuals with developmental disabilities or related conditions to support them in achieving their desired outcomes. This includes assisting with personal care routines, developing person-centered goals that reflect what is important to and for the individual, and building community connections to enhance social and volunteer opportunities.
Position Title: Summer-Sub DSP
Department: Day Services
Hours: On-Call/Temp (non-exempt)
Location: Lifeworks Services – New Hope
Compensation: $20.00 per hour

- Can pass a Department of Human Services background study and drug screen.
How you’ll be spending your time:
- Identifying, implementing, and documenting individuals’ preferences, attributes, goals, outcomes, positive support strategies, training strategies, and support needs as outlined in the Support Plan Addendum (SPA), including support provided in center-based and community settings to ensure person-centered services.
- Following person-centered plans, outcomes, and goals, including opportunities for choice and what is important to and for the individual, while supporting service development such as sharing ordinary places, building relationships, making choices, being respected, and contributing to the community where they live and work.
- Providing support and assistance to help individuals complete personal care routines, including eating, using the bathroom, and other daily needs.
- Developing and maintaining relationships and community contacts to increase access to social and volunteer opportunities for individuals.
- Attending all required trainings and team meetings.
- Documenting billable time and services provided on a daily basis.
- Following safety procedures and assisting in identifying safety needs for self and others, while performing other duties as assigned.
- High school diploma or GED required.
- Fluent in English, verbal and written.
- Basic Microsoft Office skills.
- Candidate must complete and pass a pre-employment background study; a physical and drug screen; and have access to reliable transportation for work purposes.
