Reports To: Program Manager
Supervises: None
Hours: Sunday–Thursday, 9:30 a.m. – 6:00 p.m.
Location: Santa Cruz, CA (includes work at Sobering Center as needed)
The Case Manager provides high-quality, client-centered case management services to individuals enrolled in the RENEWPath Program at Janus of Santa Cruz. This position delivers comprehensive case management interventions including assessment, goal setting, care coordination, referral processing, and crisis intervention.
Using evidence-based practices such as Motivational Interviewing, Strengths-Based Model, and ASAM, the Case Manager works collaboratively with the Program Manager and interdisciplinary team members to support client engagement and treatment outcomes. The Case Manager also processes referrals and assists with client care at the Sobering Center as needed.
This role is ideal for an individual with a bachelor’s degree in a behavioral health-related field and/or equivalent experience providing case management or care coordination. The Case Manager must demonstrate flexibility, strong organizational skills, empathy, cultural humility, and the ability to thrive in a collaborative team environment.
CLINICAL SERVICES & QUALITY PATIENT CARE- Conduct comprehensive client assessments, including ASAM brief SUD assessments, to determine appropriate levels of care such as outpatient, intensive outpatient, behavioral health therapy, and/or residential treatment
- Initiate timely and thorough referrals to appropriate SUD and behavioral health treatment programs
- Facilitate individualized goal setting and follow-through with clients using Motivational Interviewing techniques
- Respond effectively to crisis situations using de-escalation skills and Motivational Interviewing to support clients and team members
- Communicate clearly and effectively with internal Janus team members regarding client care plans
- Coordinate care with community healthcare providers and partner agencies
- Provide exceptional care coordination, including warm hand-offs and closed-loop referrals
- Accurately and efficiently document client progress and services provided
- Demonstrate the ability to self-direct, prioritize tasks, triage client needs, and manage time effectively
- Collaborate skillfully with interdisciplinary teams and community partners
- Apply an integrated, transdisciplinary approach to care
- Identify needs for and assist with access to education and training related to:
- Primary care linkage
- Integration of SUD and behavioral health care
- Chronic pain management
- Chronic medical and behavioral health conditions
- Preventive health care
- Actively engage in team-based case management activities across Janus programs
- Participate in outreach and engagement efforts with Janus clients and those referred by community partners
- Collaborate with Enhanced Care Management (ECM) and other Janus programs, including:
- Admissions
- Residential
- Opioid Treatment Program (OTP)
- Perinatal
- Outpatient
- Probation
- Attend required staff meetings, case management meetings, and external probation or case discussion meetings where client care is reviewed
- Screen and authorize potential clients for participation in the RENEWPath Program
- Attend all required trainings, supervision, and administrative meetings
- Assist with providing client care at the Sobering Center as needed
- Perform other duties as assigned by the Program Manager or Supervisor
Requirements: QUALIFICATIONS
Education & Experience
- Bachelor’s degree in a behavioral health-related field with experience providing case management or care coordination OR
- Two (2) or more years of full-time case management experience in substance use disorder treatment programs or integrated behavioral health settings (may be considered in parity with a bachelor’s degree)
Certifications & Registration
- Must be Registered or Certified through CADTP or CCAPP
- MFTi or ASW interns are encouraged to apply
Knowledge & Skills
- Knowledge of and experience with peer support modalities (e.g., WRAP, 12-Step recovery, peer support models)
- Understanding of HIPAA and 42 CFR Part 2 confidentiality regulations
- Current or recent counseling experience preferred but not required
- Strong verbal and written communication skills
- Ability to work independently while functioning effectively as part of a team
Additional Requirements
- Bilingual Spanish/English preferred but not required
- Valid California driver’s license with ability to pass a DMV check
- Ability to transport clients using a personal vehicle, as required
