Occupational Therapist - PRN at Prairieview Lutheran Homes – Danforth, Illinois

Location: Danforth, IL 60930

We are seeking a dedicated and compassionate Occupational Therapist (OT) to join our team on a PRN (as needed) basis. This position offers a flexible schedule and the opportunity to work with both inpatient and outpatient populations in a LTC nursing and memory care setting. The ideal candidate will be committed to providing high-quality care to our patients.

• Provide outpatient occupational therapy services as needed.
• Develop and implement individualized treatment plans based on patient assessments.
• Collaborate with the therapy team to ensure comprehensive patient care.
• Document patient progress and maintain accurate clinical records.
• Adhere to all facility policies, procedures, and regulatory requirements.

• Valid Occupational Therapist (OT) license in the state of Illinois.
• Current CPR certification.
• Experience working in LTC nursing facilities, memory care units, and outpatient settings preferred.
• Ability to work a flexible schedule on an as-needed (PRN) basis.
• Strong communication and interpersonal skills.
• Commitment to patient-centered care and professional ethics.
