About the Role

The Care at Home Coordinator supports the Program Administrator in leading and growing Friendsview's Care at Home (CAH) program. This role blends operational management, staff supervision, clinical coordination, and client services oversight to ensure residents receive exceptional, person-centered care in their homes.

You'll help guide caregivers and licensed nursing staff, maintain regulatory compliance, and promote a culture rooted in compassion, integrity, and excellence.

Key ResponsibilitiesProgram Operations & Compliance
  • Support organization and direction of ongoing CAH operations

  • Maintain knowledge of home care rules and regulations; ensure compliance with federal, state, and local standards

  • Identify and recommend workflow and policy improvements

  • Participate in Quality Improvement initiatives

  • Complete monthly audits per OAR 333-536-0090, including MAR reviews

  • Contribute to QAPI efforts using data to educate, coach, and improve systems

‍⚕️ Client Services Management
  • Conduct pre-service evaluations (in-home screenings)

  • Oversee admission, quarterly, and change-of-condition assessments

  • Develop, implement, review, and revise individualized service plans

  • Monitor client needs at 30 days post-admission and quarterly thereafter

  • Coordinate care with internal departments and community providers

  • Ensure accurate and compliant medical documentation

  • Conduct twice-weekly INR clinic, schedule ear lavage clinics, and provide weekly BP monitoring

  • Attend weekly Transitions Meetings and update IDT

  • Respond promptly to client and staff concerns

Staffing & Leadership
  • Supervise CAH caregivers across a 24-hour period (recruiting, hiring, scheduling, training, coaching, performance management)

  • Create caregiver schedules 1–2 weeks in advance

  • Partner with HR to ensure licensure, certifications, and documentation compliance

  • Assist with onboarding and staff development

  • Carry Home Care phone (on-call) every other weekend and some evenings

  • Fill in for direct care, including medication set-ups as taught by RN and hands-on care when needed

  • Promote worker safety and investigate safety-related incidents

Clinical & Interdisciplinary Collaboration
  • Coordinate with CAH RN to assess resident needs and provide nursing oversight

  • Ensure medications are administered per physician orders and regulatory standards

  • Communicate with physicians and outside providers; follow up on recommendations

  • Manage incident reports and oversee follow-up investigations

  • Ensure compliance with Oregon State Board of Nursing delegation standards

  • Work closely with Health Services Leadership Team on service concerns

QualificationsEducation & Experience
  • Bachelor's degree in health services–related field (healthcare, social work, gerontology, etc.)
    Relevant experience may substitute for education

  • Minimum 2 years of healthcare experience (home care or long-term care preferred)

  • Minimum 2 years supervisory experience preferred

  • Strong knowledge of care planning, medication systems, infection control, nurse delegation, and residents' rights

Skills & Abilities

  • Strong interpersonal, written, and verbal communication skills

  • Budget awareness and program oversight capabilities

  • Proficiency with computer systems (incident reporting, payroll, scheduling software, etc.)

  • Ability to maintain confidentiality at all times

Additional Requirements
  • DHS criminal background check (and fingerprinting if applicable)

  • Pre-employment drug screen and reference checks

  • Official transcripts (if applicable)

  • Valid driver's license, insurance, and acceptable driving record (if driving required)

  • Documentation to verify authorization to work in the U.S.
