Occupational Therapist, Dementia Team

With the support of the community, Hospice of the Valley’s Dementia Care and Education Campus is an innovative and comprehensive resource for those living with Alzheimer’s disease and other dementias. The Campus will provide full-spectrum care for persons living with all stages of dementia and support for their families. A 12-person Assisted Living Center, a 10-person Hospice Inpatient Care Home, an Adult Day Center, and a Child Center will provide superb and creative person-centered care, and a Community Education Center will educate and inspire physicians, nurses, social workers, caregivers, families, and the community to improve quality of life for persons living with dementia.

Position Profile

This role serves as a resource for occupational therapy teaching and service for persons living with dementia at the Dementia Care and Education Campus, in the Supportive Care for Dementia program, and for hospice patients living with dementia. Contributes to providing therapy to optimize cognitive and physical functioning of patients, assists families in optimizing home environments, and teaches meaningful engagement for patients and their caregivers. Provides education to professionals and community members about occupational therapy, including optimization of physical and cognitive abilities, home adaptations, and engagement. This role will work at least one weekend each month and one early evening each week to oversee engagement in the DCEC Assisted Living facility

Responsibilities

  • Provides skilled occupational therapy services to persons living with dementia in any of the Hospice of the Valley’s programs.
  • Responsible for teaching O.T. students, other professionals, caregivers and community members about occupational therapies for persons living with dementia.
  • Develops, maintains, and enhances a specialized knowledge base and professional skills regarding occupational therapy for persons living with dementia.
  • Adheres to scope of practice/HOV standards and facilitates continuously improved process/services.
  • Maintains and enhances professional skills.
  • Adheres to high standards of personal professional conduct.

Minimum Qualifications

  • Must be a graduate of an accredited program in Occupational Therapy
  • Minimum two years’ experience as an occupational therapist.
  • Dementia, Home health, Rehab and/or hospice experience preferred.
  • Current Arizona License of Occupational Therapy.
  • CPR Certification.
  • Valid Arizona driver’s license and insured automobile.

Preferred Qualifications

  • Knowledge of dementia preferred.
