JOB DESCRIPTION SUMMARY
Acacia Hospice of The Desert's Director of Patient Care Services is responsible for the overall direction of hospice clinical services. As our hospice clinical leader, this role establishes, implements and evaluates goals and objectives for hospice services that meet and promote the standards of quality of patient care and Acacia's core values.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Oversees all hospice patient care services.
- Provides guidance and counseling to coordinators and Clinical Supervisors to assist them in continually improving all aspects of hospice care services.
- Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care.
- Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel.
- Provide oversight of all patient care services and personnel.
- Works alongside Clinical Supervisor to manage clinical teams and planning.
- Reviews patient referrals prior to admission.
- Oversees the maintenance of patient records, statistics, reports and records.
- Responsible for the maintenance of supplies and equipment.
- Plans and implements inservice and continuing education programs.
- Assist with patient visits and coverage as needed.
- Assist other departments in all processes related to patient care.
POSITION QUALIFICATIONS
- Valid & active California RN license.
- Valid & active California Driver license.
- Experience working with Homecare Homebase (HCHB) EMR.
- Must have prior experience as DPCS or Hospice Clinical Supervisor.
- Bachelor’s degree in Nursing from an accredited program by the National League for Nursing.
- Three years of management experience within the last five years in a hospice agency, at least one year of which was a supervisory or administrative capacity.
- Knowledge and expertise in clinical decision-making and Quality Management review and evaluation.
- Knowledge of Medicare Conditions of Participation.
- Has excellent observation, verbal and written communication skills.
