About This Position
Upstate Family Health Center, a Federally Qualified Health Care Center (FQHC) located in the heart of the Mohawk Valley in Utica, NY, is seeking passionate and dedicated Licensed Practical Nurses to join our team. The clinic is open 8:00am-6:00pm.
Schedule: 4-day work week, varied Monday - Friday (10 hour shifts)

Position Overview
As an LPN at Upstate Family Health Center, you will function under the supervision and guidance of the Medical Provider or Registered Nurse. You will participate in the multidisciplinary process of providing care and treatment to patients, offering clinical support and technical assistance to professional medical staff. Using a team-based care model, you will form effective partnerships with patients, their caregivers/families, community resources, and their primary care physician.
Essential Roles and Responsibilities
Prepare medical treatment rooms and set up patient care equipment.
Prepare patients for tests or treatments.
Use sanitation practices in healthcare settings.
Assist in examining or treating medical patients as needed.
Follow medical office procedures.
Record medical history or data.
Weigh patients and take vital signs.
Collect specimens from patients, including blood draws (venipunctures), capillary blood draws, urine, and stool specimens.
Administer vaccines and other medications as directed by provider orders.
Prepare samples for laboratory testing, analysis, or microscope.
Follow patient care procedures.
Sterilize or disinfect instruments.
Maintain patient confidentiality and adhere to HIPAA guidelines.
Manage patient care in collaboration with other health professionals to make treatment decisions and attain health goals.
Provide education and coaching to patients and families for successful self-management of chronic diseases, including medication reconciliation and management.
Obtain prior authorization for care through clients' insurance carriers as applicable.
Universal Duties
Attend department meetings and mandated continuing education/training programs.
Comply with all pertinent regulatory and agency regulations and operating standards.
Establish a positive work environment that fosters communication between supervisors and employees.
Participate in team activities, including meetings, trainings, and committees.
Perform other duties as assigned.
Education and Experience Requirements
Minimum of a High School Diploma PLUS specialized NYS LPN Licensure.
BLS AHA Certified Preferred.
1-3 years of experience working in a clinical or community health center.
1-3 years of experience working with chronic disease patients.
1-3 years of experience working with EMR systems and data reporting.
Intermediate PC skills (Excel, PowerPoint, Word, Outlook, SharePoint).
Ability to act calmly and effectively in an emergency situation.
Flexibility to respond to changing demands.
Exceptional communication and interpersonal skills.
Knowledge, Skills, and Abilities (KSAs)

Work Environment
Blood Borne Pathogen Category (OSHA) – Category I: Job-related tasks involve exposure or potential exposure to blood, body fluids, or tissue and tasks are a condition of employment.
Physical Demands
The physical demands include the ability to type and operate a personal computer, bend, stoop, crouch, kneel, twist, balance, and lift up to 60 pounds.
Establish and maintain effective work relationships with coworkers and patients.
Maintain regular attendance and carry out a variety of oral and written instructions.
Work effectively under deadlines and address emergent issues promptly.
