About the role

  • The Supportive Housing Case Manager will provide intensive supportive services to tenants, chronically homeless adults, or those who are at risk of being homeless. Individuals who are living with co-occurring substance use and mental health disorders (CODs) help them develop skills to build meaningful, purposeful lives in the community through housing and case management. S/he will engage clients where they are to help them develop and build the basic skills they need to obtain and retain permanent housing.
    Responsibilities include but are not limited to developing individual service plans; assisting tenants with obtaining benefits; providing supported referrals to external providers, including primary care, mental health and substance abuse treatment, education and employment opportunities, and childcare services; and supporting the development of tenants’ activities of daily living skills. The Supportive Housing Case Manager supports clients through HUD Coordinated Entry process, provides housing stabilization services and identify clients at risk for relapse or eviction and work with MDT to provide crisis intervention. Housing stabilization services will be provided to support individuals in maintaining sobriety/recovery; gaining residential stability; increasing their skills and income and obtaining a greater level of self-sufficiency.

What you'll do

  • Conduct global assessments/biopsychosocial with each resident and develop an Individual Service Plan (ISP) within two days of lease signing. ISP should identify resident’s strength, needs, preferences, and desires with clear, measurable goals and specific action that align with HUD Service Plan for each project
  • Conducts resident orientations including review of tenant rights and responsibilities; and providing information on services available on campus.
  • Provides case management and supported referrals to services including benefits and entitlements; legal assistance; primary and specialty medical care, including children’s healthcare and dental services; HIV/AIDS supports; domestic violence support; parenting supports and education; meal services; outpatient mental health and substance abuse treatment; education, employment, and/or volunteer opportunities; and peer support and other recovery oriented groups.
  • Conduct home visits as needed accompanied by Associate Director of Recovery Support Services and Or Associate Director of Operations to ensure tenant safety and engagement in support services.
  • Coordinate internal and external services needed to meet service plan goals and increase residents’ ability to maintain housing and engagement in recovery supports.
  • Provide mentoring and support in activities of daily living, such as financial management, meal preparation and nutrition, personal hygiene and fitness, personal and fire safety, household management, and conflict resolution skills that will help residents to maintain permanent housing
  • Review each resident’s progress in achieving goals and objectives weekly and revise service plan accordingly.
  • Engage in outreach with other external providers and organizations to develop a comprehensive network of services available for tenants
  • Facilitate recreation and socialization opportunities for residents as well as opportunities for community engagement. Weekly tenants’ meetings, groups and/or workshop with a quarterly social/recreational.
  • Provide interpretation at housing, medical, legal, and other appointments to support effective communication for residents.
  • Provide transportation assistance to and from appointments, including aftercare treatment and support services.
  • Provide linkage to 24-hour crisis prevention and intervention services
  • Track resident participation and goal attainment and maintain all tenant records in compliance with DPH and HUD regulations.
  • Completes any compliance reporting as required
  • Responsible for maintaining all documentation of activities including logs, case notes, and HMIS database records, including client intakes, program entry and program exit
  • Assists in acquisition of furniture and supplies, as needed.
  • Provide housing search support for tenants seeking to relocate to a higher level of independent living.
  • Support Property Manager in coordinating quarterly tenant meetings and ensuring resident participation.
  • Participate in Multi-Disciplinary Team (MDT) and Office Based Addiction Treatment meetings to coordinate with internal staff regarding services delivered to residents through the Familias Unidas Outpatient Clinic and ensure service plan goals align with treatment plan goals.
  • Identify clients at risk for relapse or eviction and work with Multi-Disciplinary Team (MDT) and present high-risk client to provide crisis intervention
  • Participate individual and group supervision meetings, staff meetings, and other meetings as assigned based on role
  • Assist with applying for health insurance and other benefits
  • Provide intensive support around housing search, placement, and stabilization to ensure housing retention
  • Provide group and individual housing education services, such as housing resources, CORI advocacy, tenant rights, etc.
  • Provide clients with ongoing education regarding substance abuse, mental health, HIV/AIDS, Hepatitis, tobacco, nutrition and exercise and other health and wellness information
  • Collaborate with the Director of Programs to address and resolve resident complaints.
  • Work at least two evenings per week to ensure availability for resident needs and program activities.
  • Provide Housing search support and Case Management support to Outpatient clients as needed.
  • Perform other duties as required to support program goals and operations.

Qualifications

  • Bilingual Spanish/English required.
  • Minimum bachelor’s degree preferred
  • Certified Alcohol and Drug Counselor/Certified Alcohol Counselor required or willingness to obtain within 1 year of hire.
  • Minimum 2 years’ experience providing services to people struggling with CODs and/or homelessness
  • Knowledge of HUD Coordinated Entry Process and regulations
  • Knowledge of appropriate community resources
  • Familiar with social, health and cultural issues specific to the Latino population
  • Ability to work as a team member & independently
  • Knowledge of substance abuse field
  • Strong interpersonal skills and crisis management skills
  • Understanding of relapse prevention and harm reduction techniques
  • Must be able to effectively use computer programs, including web-based database system
  • Valid Massachusetts Driver’s License
  • Ability to meet the physical and training requirements of the position
  • Experience working in an advocacy role for children and families
  • Experience with homelessness, mental illness, domestic violence, HIV/AIDS, and child case management
