Family Services Facilitator – Sawyer County Health and Human Services

Sawyer County is seeking a dedicated and compassionate individual to join our Children, Youth, and Families (CYF) Unit as a Family Services Facilitator. Under the direct supervision of the CYF Supervisor, this position provides essential support and services to children and families from diverse cultural, social, and economic backgrounds.

Primary Responsibilities:
The Family Services Facilitator will focus on supporting supervised family interactions and providing a range of services including:

  • Crisis intervention
  • Risk and safety assessments
  • Parenting education and guidance
  • Instruction and assistance with household tasks
  • Budgeting and money management support
  • Child development education
  • Connection to community resources to meet basic needs
  • Transporting youth and families as needed. This may require long-distance travel within Wisconsin

In addition, this position will primarily support Child Protective Services (CPS) Access, which includes:

  • Answering and documenting intake calls regarding suspected child abuse or neglect
  • Meeting in person with reporters to gather detailed information
  • Supporting caseworkers in both CPS and Youth Justice (YJ) units as needed

Qualifications:

  • High school diploma or equivalent required (Associate degree in Human Services or related field preferred)
  • Prior experience in social services or community outreach is highly desirable
  • Strong interpersonal and communication skills
  • Demonstrated empathy, patience, and a genuine desire to help others
  • Familiarity with community resources
  • Excellent organizational, time management, and computer skills
  • Ability to maintain confidentiality and manage sensitive information appropriately
  • Valid Wisconsin driver’s license

Compensation and Benefits:
This is a full-time, 40-hour per week position with a starting wage of $22.91/hour. Sawyer County offers a competitive benefits package, including:

Job Description


Title: Family Services Facilitator

Department: Health and Human Services Department

Reports to: Children, Youth and Family Services Supervisor

Position Purpose:

Under the direct supervision of the Children, Youth and Family (CYF) Supervisor, the Family Services Facilitator will provide services to a population of children and families with diverse cultural backgrounds and various levels of education, social, and economic skills. The following service types will be directed at supervised family interactions: crisis intervention, assessment of risk and safety, parent guidance and education, instructing, demonstrating and performing household tasks, money management/budgeting, child guidance and development, resources to meet basic needs, and parenting skills.

Additionally, this position will primarily support CPS Access. This includes taking phone calls and meeting with reporters in person to gather needed information on children who are alleged to be abused. This position will also support caseworkers in both the CPS and YJ areas.

Essential Functions:

· Receives and documents reports of alleged maltreatment from the community.

· Assesses individual and family strengths and needs. Partners with families to establish a service plan that will meet identified needs that are in conjunction with the assessment and service planning of the assigned social worker specialist. Acts to resolve unsafe family conditions and assists in achieving immediate goals of the family with an understanding of and sensitivity to past traumatic experiences.

· Teaches and acts as a role model for child guidance skills and knowledge, household and money management, meal planning and preparation, problem-solving skills, accessing community resources, employment, housing and self-esteem building.

· Provides supervised visitation which includes transportation of both children and parents.

· Provides support to all case managers in the CYF Unit.

· Must possess a valid WI Driver’s license

Additional Essential Duties:

· Provides information and emotional support to parents, referring and connecting families to needed services and encouraging and utilizing the use of individual and family support systems.

· Addresses and resolves conflict and manages stress in a professional manner, including actively engaging in professional and personal activities which help mitigate the impact of secondary traumatic stress.

· Applies Trauma Informed Care principles and practices, and demonstrates a strong commitment to fostering a positive, healthy work culture.

· Provides court testimony relative to assigned cases.

· Performs as a team member with other professionals and paraprofessionals evaluating service plan appropriateness, changes needed in services, and progress toward stated objectives in both verbal and written manner.

· Provides parenting skills training on an individual or group basis to a diverse population.

· Maintains family service records on assigned cases per agency policy and as prescribed in case service plans.

· Maintains competency in program areas through further education and in-service training opportunities.

· Ensures confidentiality regarding families and other business is maintained at all times.

· Ensures compliance with federal, state and county mandates.

· Ensures that paperwork is completed efficiently and in a timely manner.

· Ensures that families are treated courteously and provided or referred for needed services.

· Ensures safety through the implementation of safety plans developed by the Social Worker/caseworker.

· Performs other duties as assigned.

Education and Experience

· High School diploma or equivalent (Associate degree in social work or related field preferred but not required); previous experience in a social service or community outreach role.

Knowledge Skills and Abilities

· Possesses good knowledge of household management, child development, parenting skills, money management, problem solving, and understanding of human behavior.

· Working knowledge and understanding of Trauma Informed Care, and commitment to providing trauma informed services.

· Ability to thoroughly assess family strengths and needs with sensitivity to past traumatic experiences; and incorporate these into appropriate goals developed in partnership with families.

· Knowledge of available appropriate community resources

· Ability to communicate effectively both verbally and in writing.

· Ability to organize, assess and prioritize client and job responsibilities.

· Ability to accept and utilize supervision and directives while working within an agency team structure.

· Basic computer skills. Ability to utilize computer programs to document case activities and use for education and training with clients.

· Must have patience and ability to give step by step instructions to accomplish simple tasks.

· Ability to provide confidential services to families and establish a professional and friendly rapport with families.

· Ability to effectively address and resolve conflict and to manage stress in a professional manner.

· Ability to work flexible hours outside of normal agency hours to ensure the ability to meet the needs of parents and children.

PHYSICAL AND MENTAL ABILITIES REQUIRED TO PERFORM ESSENTIAL JOB FUNCTIONS

· Ability to exercise the judgment, decisiveness, and creativity required in situations involving the direction, control, and planning of an entire program or multiple programs.

Physical Requirements

· Ability to sit or stand for extended periods while performing duties. Ability to drive. Ability to work out in the field. Ability to perform physical demands such as seeing, hearing, and color perception to make judgments both in the office and out in the field.

· Ability to operate a variety of office equipment including computers, keyboard, telephone, fax machine, calculator/adding machine, and photocopier. Ability to perform movements requiring moderate skill, such as typing. Ability to sustain prolonged visual concentration. Ability to exert light physical effort in sedentary to light work which may involve some lifting, carrying, pushing, and pulling.

Environmental Adaptability

· Ability to work under safe and comfortable conditions where exposure to environmental factors such as repetitive computer keyboard use poses a very limited risk of injury.
