The position involves direct and indirect patient care for a population of patients between the ages of 18 and older. Age specific experience and/or special training and/or expertise is required to serve this population.

Requirements: POSITION SUMMARY

Using discretion and independent judgement and under the general supervision of the hospital CEO and CNO, the Director of Nursing organizes and administers the Nursing Services at the hospital. The DON implements standards of care, policies, and procedures as established by the company. The DON insures timely, accurate, complete documentation by the clinical services on company approved forms. The DON ensures that all clinical regulatory requirements are met. This position is responsible to fully implement standard clinical programs: Pulmonary-Ventilator Weaning, Complex Wound Care, and Complex Medical including Infectious Disease and Renal, and Brain Injury Recovery. The DON steers progressive clinical excellence through staff development and implementation of the Core Curriculum, competency validation through EDUCATION testing, and the Clinical Advancement Program. The DON is an integral member of the leadership team and shares responsibility for employee engagement, recruitment and retention. Clinical cost control is an essential part of the role. This includes implementation of the wound formulary, bed algorithm and purchasing and renting supplies through standard vendors. Safe staffing is also a key element of this position. The DON ensures that all staff function within the job description and licensure. Coalinga Regional Medical Center standards related to the role of the RN must be implemented. Coalinga Regional Medical Center staffing guidelines are required. The DON is responsible for census through: clinical competency and reputation, excellent physician relationships and a culture of professionalism and customer service.

The DON will administer and follow all Coalinga Regional Medical Center HR policies with particular attention to uniform, consistent and fair implementation of the attendance policy, policy on progressive discipline, suspected drug diversion policy, and policies related to a positive work environment and professional appearance. Significant clinical initiatives such as National Patient Safety Goal Implementation, VAP and CVC line bundles, Hourly Rounds, Delirium, SBAR-Change in Condition, Sepsis Screen are also required. This position does provide direct patient care to adolescent, adult, and geriatric patients.

POSITION QUALIFICATIONS

This position requires a Bachelor Degree in Nursing from an accredited school of nursing. The position requires at least two (2) years of progressive work experience or equivalent in a management or administrative position, and current licensure to practice as a Registered Nurse in the State. The DON must show evidence of active participation in continuing education in Nursing Executive Management and Health Care Administration.

This position requires knowledge and demonstrated capacity for leadership skills in the design, implementation, and evaluation of patient care programs that are coordinated with the health care of the patient. The development of staff and management of personnel in an environment which enhances the highest standards of care is also required. This position requires fiscal and quality accountability necessary to meet the standards and goals of this health care organization. Participation in health associations and educational programs indicative of professional growth is required. BLS and ACLS required.

DUTIES AND RESPONSIBILITIES

I. ASPECTS OF THE DEPARTMENT OF CLINICAL SERVICES, IN ADDITION TO ALL REQUIREMENTS OF THE REGISTERED NURSE JOB DESRIPTION

A. Provides leadership, support and participation in Hospital QAPI meetings

B. Maintains safe environment through strict enforcement of all Infection Control policies and monitoring of effective and safe housekeeping practices

C. Implements Coalinga Regional Medical Center standards related to staffing:

1. No RN on LTAC unit can be assigned to or cover for more than 5 patients 2. 100% RN

3. RT- 1:4-6 vents

4. Rehab according to average census

5. Written assignments

6. Report between nurse and nurse aide

D. Implements core curriculum and competency validation through education.

E. Meet regulatory requirements for documentation.

F. Implements core programs.

G. Implements significant corporate clinical initiatives: VAP and CVC line bundles, NPSG’s, hourly rounding, shift huddle, Delirium screen, Sepsis screen, skin care bundle, fall reduction bundle and others as directed by Coalinga Regional Medical Center.

A. Follows and administers HR policies. Consults with HRC and HRD as needed. Attention given to:

1. Attendance policy

2. Professional appearance policy

3. Progressive discipline policy

4. Positive work environment policies and practices

5. Suspected drug diversion guideline

B. Cooperates in the formulation of policies and practices established by the Hospital Administrator/CEO. Defines and interprets these to the Department of Nursing

C. Plans with Medical Staff and Department heads for coordination of services and activities.

D. Meets all new patients in timely way

E. Enforces grievance policy

F. Holds monthly staff meetings accessible to all shifts

G. Present on night shift monthly

H. Develops charge nurses into a clinically and administratively effective team

I. Uses Clinical Observation tool four times a year (quarterly) for all clinical employees to provide ongoing assessment and feedback of performance

J. Clinical rounds daily

K. Controls cost: purchasing and rental agreements, bed algorithm, wound formulary

L. Effectively functions with leadership team

M. Meets CMS and Accreditation requirements

N. Keeps the Hospital Administrator/CEO informed of plans, equipment, systems, policies and procedures.

O. Relates with appropriate boards, community, and governmental agencies.

P. Develops a working relationship with professional organizations and groups and acts in leadership capacities.

Q. Represents the hospital to professional community groups and actively participates within and outside the hospital to promote its reputation, educational, and research activities related to the quality of services.

II. PROVIDES A LEVEL OF COMPETENCY TO PATIENTS AGES 18 AND OLDER.

A. Adults:

1. Involves them with care planning by meeting any needs they identify during assessment.

2. Identifies their normal patterns of living and strives to attain them.

B. Geriatric:

1. Strives to obtain understanding by speaking distinctly and moving slowly.

2. Involves family as much as possible.

3. Explains all procedures, provides for adequate rest periods, and listens to their concerns and fears.

4. Assesses sight and hearing.

III. MAINTAINS LICENSURE, CONTINUING EDUCATION REQUIREMENTS, AND CPR CERTIFICATIONI. DISPLAYS A “CUSTOMER SERVICE” ATTITUDE TOWARD PATIENTS, VISITORS, STAFF MEMBERS, AND REFERRAL SOURCES OF ALL AGES, AT ALL TIMESII. FOLLOWS SAFETY, INFECTION CONTROL, AND OTHER GUIDELINES AS REQUIREDIII. PROMOTES AN ETHIC OF CONTINUOUS QUALITY IMPROVEMENT IN ALL MATTERS ASSOCIATED WITH THE POSITION, AND SHALL DOCUMENT SPECIFIC QUALITY ASSURANCE IMPLEMENTATIONIV. PERFORMS ALL OTHER TASKS AND DUTIES AS MAY BE ASSIGNEDV. DRESSES AND PRESENTS SELF IN A PROFESSIONAL MANNER AT ALL TIMES. ADHERENCE TO THE CORPORATE APPEARANCE POLICY IS REQUIRED.VI.
