Job Description

Palm is seeking a compassionate and patient‑focused Social Worker to support individuals with complex social, emotional, and behavioral needs that impact their health outcomes. This role is responsible for assessing patients, providing crisis intervention, coordinating care, and connecting patients and families with appropriate internal and community-based resources.

The Social Worker collaborates closely with providers, care teams, and external agencies to support whole‑person care and reduce barriers to effective treatment.
Duties & Responsibilities:
  • Assess patients’ psychosocial needs and gather relevant information through in‑person and telephone interactions.
  • Provide crisis intervention and emotional support to patients and families experiencing acute or chronic stressors.
  • Assist patients in accessing appropriate services, benefits, and community resources.
  • Educate and support patients and families regarding diagnoses, treatment plans, and social implications of illness.
  • Develop and contribute to individualized treatment and care plans.
  • Coordinate referrals to internal departments and external agencies, social services, and support organizations.
  • Encourage patient participation in beneficial programs and activities.
  • Maintain accurate, timely documentation and prepare reports as required.
  • Participate in training, clinical supervision, team meetings, and case reviews.
  • Travel to Palm centers as needed to support patient care and operational needs.
  • Other duties as assigned.

Qualifications & Experience:
  • Bachelor degree in psychology, social work or related field.
  • Experience working with clients that are affected by issues including neglect, domestic violence, mental health and parental substance abuse.
  • Ability to give full attention to what social service clients are saying, understand the points being made, ask questions and not interrupt inappropriately.
  • Awareness of others’ reactions and understanding why they react as they do.
  • Ability to judge the relative costs and benefits of potential social service and health care actions and choose the most appropriate one.
  • Comfortable using a computer for various tasks.
  • Proficient in Word, Excel, Outlook, and PowerPoint.
  • Ability to stay calm and empathize with clients when they are upset.
  • Licensed clinical social worker.
  • Able to develop a treatment plan.
  • Knowledge of crisis intervention.
  • Valid driver license.
