About the Role – Intake & Screening Specialist
The Intake & Screening Specialist serves as the first point of contact for members entering StrongSteps Behavioral Health programs and services. This position is responsible for conducting Health-Related Social Needs (HRSN) screenings, eligibility assessments, intake coordination, and service referrals for individuals seeking support with housing, food insecurity, transportation, care management, and other community-based services.
The Intake & Screening Specialist plays a critical role in ensuring members are connected to the appropriate level of support quickly and efficiently while maintaining accurate and compliant documentation required for Medicaid, SCN, and HRSN service delivery. This role works closely with Care Managers, Housing Specialists, and administrative staff to ensure smooth onboarding and continuity of care.
Responsibilities- Conduct intake interviews, screenings, and eligibility assessments
- Review and collect required member documentation
- Coordinate onboarding into care management and support services
- Maintain accurate and timely documentation in agency systems
- Respond to incoming referrals and inquiries
- Schedule follow-up appointments and service coordination
- Communicate with members, providers, hospitals, and community partners
- Ensure compliance with agency, Medicaid, and HRSN documentation requirements
- Assist with referral tracking and intake reporting
- Support member engagement and connection to community resources
- Strong communication and organizational skills
- Experience in behavioral health, social services, healthcare, or community outreach preferred
- Ability to manage multiple tasks and maintain accurate documentation
- Comfortable working with vulnerable populations and individuals experiencing social care barriers
- Basic computer and documentation skills required
- Knowledge of Medicaid, care coordination, or HRSN services preferred
- Full-Time Position
- Hourly, Non-Exempt
- Starting Pay: $20/hour
