Licensed Clinical Social Worker

  • JOB SUMMARY
    • The Licensed Clinical Social Worker at KD supports the needs of patients served in the Outpatient Behavioral Health office through provision of clinical social work services, including screening and brief intervention, assessment, diagnosis, treatment planning and coordination, and monitoring of mental health needs. He/she will offers a mix of traditional outpatient counseling services with onsite behavioral health consultation services. He/she serves as a member of the care team, available to serve all patients receiving primary medical, oral or behavioral health care in the clinic. This will include, but is not limited to, the assessment and treatment of individuals, families, and groups, as well as crisis intervention and engaging with patients as they present for services. The LCSW will collaborate with other involved providers to achieve the best care for patients and will strive to promote whole person improvement of health outcomes.

  • DUTIES AND ESSENTIAL FUNCTIONS
    • · Participate as an active member in the patient's outpatient care team.

      · Assisting colleagues in managing behavioral health crises in the clinic

      · Maintaining a visible presence in the clinic when not engaged in therapy sessions to promote collaboration between disciplines;

      · Utilizes EPIC and care team huddles to provide insight and direction related to behavioral health needs.

      · Utilize clinical interventions from a variety of theoretical bases to treat patient mental health or substance use conditions, including enhancement of motivation to address mental health, physical health or substance use conditions, development of diagnosis, treatment plan, and plan for termination of treatment episode.

      · Assign appropriate diagnosis for a range of behavioral health conditions, including substance use disorders.

      · Engage patient in identification of specific treatment goals and interventions/supports needed to achieve these goals.

      · Utilizes standardized screenings and assessments in accordance with organizational clinical practice guidelines and quality improvement initiatives.

      · When applicable and as indicated by patient need, provide individual, family, couples and/or group therapy to patients.

      · Provide care management services to patients with behavioral health and substance use needs, including follow up on referrals to appropriate outside or specialty behavioral health resources and collaboration with other services to ensure that patient psychosocial needs are addressed.

      · The LCSW will ensure all records are adequately detailed, problem-oriented, timely, truthful, clear and accurate to allow good continuity of patient care and accurate billing (coding of diagnoses, services provided, and charges).

      · The LCSW clinical notations should facilitate another provider's assuming follow-up care.

      a. Notations should be completed on the day that services are provided.
      The documentation should reflect medical necessity of service, interventions utilized, progress in treatment or change in treatment plan if patient is not progressing.

      · Participation in the MSW programs in student practicum rotations and supervision of KD CSW’s seeking LCSW degrees.

      · Participate in staff meetings and quarterly provider meetings as requested.

      · Assist with other duties or in other departments as instructed by supervisor.

  • EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
    • Minimum requirement:
      • Licensed Clinical Social Worker
      • Maintains unrestricted Kentucky LCSW license and meets ongoing licensure requirements.
      • Achieves and maintaintains Non-Violent Crisis Intervention Certification
    • Preferred qualifications:
  • WORKING ENVIRONMENT
    • Constantly works near moving mechanical parts
    • Constantly exposed to blood and/or body fluids and infection.
    • Occasionally exposed to toxic or caustic chemicals
    • Occasionally exposed to risk of radiation
    • The noise level in the work environment is usually moderate.
  • PHYSICAL DEMANDS
    • Occasionally required to maintain in a stationary position (standing or sitting).
    • Frequently required to walk and move about.
    • Frequently required to operate a computer and other equipment.
    • Frequently required to use fine motor skills to handle and operate instruments/devices.
    • Constantly required to lift and/or move items up to 10 pounds.
    • Frequently required to lift and/or move items up to 50 pounds.
    • Occasionally required to lift and/or move items up to 100 pounds.
    • Constantly required to communicate with patients and other care team requiring talking and hearing.
    • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
    • Occasionally required to position self in order to meet patient needs.
