Department Overview

OHSU Health Primary Care at South Hillsboro is part of HMC healthcare and OHSU School of Medicine. We provide preventive, primary and specialty care for you, your children, your spouse, your parents and your grandparents. We provide all that family medicine has to offer - from maternity care through geriatrics. Family Medicine strongly supports the comprehensive scope to provide the highest level of care for our community. The clinical work includes but is not limited to all ages of life including newborn to end of life, taking care of those who are pregnant, and supporting patients with substance use disorder including medication-assisted treatment and gender affirming care. This position supports both clinical and education missions within the Department of Family Medicine. The Behavioral Health Consultant (BHC) position is a behavioral health clinician who operates as a core member of the integrated care team within Family Medicine at Primary Care at HMC South Hillsboro.

This position involves a high degree of clinical complexity and requires advanced-level generalist skill in delivering evidence-based interventions targeted to improve health and wellbeing for a large variety of patients across the lifespan. The BHC position serves as an advocate for integrated behavioral health within the primary care setting. This position also serves as the behavioral health team lead, providing direct support to the team through the development and management of work flows, management of schedules, direct clinical consultation and functions as a BH representative at clinic-based meetings.

In addition to the clinical role, this position provides educational support to the Hillsboro Medical Center Family Medicine Residency through the development and delivery of behavioral science curriculum. This includes delivering didactic material, providing teaching in the clinical environment, participating in resident recruitment and interviews, participating in evaluation of residents, and working closely with the Residency Program Director to meet the behavioral science educational needs of all HMC family medicine residents.

Function/Duties of Position

Curriculum Development and Direct Teaching

FM Residency

• Lead the design and implementation of longitudinal curricula concerning identification and treatment of mental health disorders, patient-centered communication skills, emotional content of clinical care, physician wellness, and integration of psychosocial assessment into primary care visits and coordination of care with local mental health and social safety net resources. This will include didactic lectures and innovative teaching processes such as:
• (1) Role play: design patient scenarios through which residents can practice communication skills with each other.
• (2) Simulation Learning: assist in planning, carrying out, and evaluating the following training Observed Structured Clinical Examination simulated encounters to teach and assess family medicine residents communication skills with patients who have mood disorders.
• (3) Video Review: participate in video review of resident encounters to assess counter-transference processes and communication skills.
• (4) Experiential Learning
• (5) Clinical teaching: Shadowing, co-facilitating visits, consultation
• Present curriculum and ongoing educational material in the behavioral science to residents, medical students, faculty and staff
• Design and present curricula in collaboration with other core faculty on the subjects of wellness, professionalism, health equity and social justice.
• Instruct residents and medical students on working with mental and behavioral health professionals in the patient centered medical home and during clinical encounters.
• Lead and organize Balint group and/or support groups for FM residents. Identify appropriate support group leaders for junior residents and provide mentoring as needed for these facilitators.
• Organize video review and facilitate weekly reviews with residents and physician faculty.
• Attend residency conference as core faculty on monthly basis and demonstrate incorporation of psychosocial thinking in various aspects of biomedical training.
• Participate in residency recruitment activities, including file reading and scoring, interviewing all candidates and participating in group debriefing of interviews.
• Attend residency leadership meetings while providing relevant input using a psychosocial lens.
• Serve on residency committees as needed and provide guidance to resident participants when necessary.
• Work with the program director and associate residency directors in the remediation of residents in difficulty. Serve on academic advancement committees as needed to provide unique perspective on developing coping strategies, social and structural supports during residency. Serve as an effective model of how to communicate with resident learners who may struggle with feedback or remediation efforts.
• Act as mentor or capstone advisor to residents performing research or QI projects in the behavioral sciences. Meet on a periodic basis with residents to provide guidance, feedback and ongoing support for their scholarly work.

Clinical

• The Behavioral Health Consultant (BHC) position is a behavioral health provider who operates in a consultative role within Family Medicine clinics, works to support a smooth interface between primary care and specialty services, and provides point of care behavioral interventions to the primary care patient population.
• Clinical duties at any of the OHSU Family Medicine Clinics as follows:
• Work with the Primary Care Providers and other office staff to recognize and address psychological symptoms and conditions using the bio-psychosocial model.
• Act as a behavioral health consultant to the primary care team.
• Work within the integrated primary care model of service delivery with Warm Hand-Off based referrals. Perform assessment, management, and triage of psychological and social problems in clinic patient population.
• Work with health providers to monitor and manage “at risk” and “chronic” patients.
• Support provider decision making and builds on provider-patient interventions.
• Provide effective triage of patients in need of specialty behavioral health.
• Coordinate and work with other team members to ensure the efficient and effective flow of patients through the department.
• Establish and maintain links with other related professional services to ensure that necessary clinical and social resources are provided for patients
• Develop and maintain a trusting and partnering relationship with assigned patients in order to assist in the success of treatment.
  Obtain complete and accurate information in a courteous, respectful and culturally sensitive manner.
  Attend clinic staff meetings, training sessions, and planning and problem-solving sessions.
  Patient notes to be written in EMR in order to provide adequate information for other health care providers.

Administrative

• Work with department and school of medicine educational leadership to develop new and innovative learning opportunities for resident and medical student clinical experiences with regard to behavioral science and social medicine.
• Create and coordinate experiences with internal and community partners to deepen residency education regarding behavioral health services.
• Serve as a mentor for members of the behavioral health team who require additional support.

Required Qualifications

• Required: Master’s Social Work (MSW); LCSW, Doctoral Degree in Clinical Psychology (PsyD or PhD) and currently licensed in the state of Oregon
• Experience in educational setting with curriculum design.
• Experience in teaching family medicine residents (preferred)
• Fellowship trained in medical education or primary care psychology (preferred)
• Clinical experience working in a primary care medical setting, preferably with “warm hand-off” model of clinical behavioral health integration.
