Responsible for the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice.

I. Major Responsibilities:

1. Utilizes the Nursing Process for the delivery and management of patient’s care from Admission to Discharge, appropriate to the developmental age of the patient population.

2. Performs patient assessment/reassessment according to standard procedure.

3. Develops and coordinates individual plan of care including discharge planning.

4. Directs, supervises, coordinates and evaluate nursing care within assigned clinical setting.

5. Delegates appropriately to others in accordance with job description and competency level.

II. Position Qualifications:

License/Certification/Education:

Required:

1. Current Massachusetts Nursing licensure and registration.

2. BLS is a requirement for every RN unless advance cardiac life support (ACLS) certification is current, in addition to any unit specific certifications and competencies.

Preferred:

1. Bachelor of Science degree in Nursing highly preferred.

Experience/Skills:

Required:

1. Thorough knowledge of the discipline of Nursing.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

III. Physical Demands and Environmental Conditions:

Work is considered medium. Position requires work indoors in a patient care environment.

Additional Qualifications

ADDENDUM FOR BARRE HEALTH CENTER OBOT NURSING:

The following descriptions are specific to the Barre Health Center Office-Based Opioid Treatment (OBOT) RN position. They are in addition to the Barre Health Center Registered Nurse Position Description.

This position requires the RN to function independently in the planning, development, and implementation of skilled nursing care and care management services for patients at risk for or diagnosed with Opioid Use Disorder.

The OBOT RN is the first contact with patients entering the medication assisted treatment (MAT) program and must be able to make independent decisions regarding the initiation, titration, tapering, and/or discontinuation of medications. Therefore, the nurse must have a strong background in caring for this vulnerable patient population.

The OBOT RN is also responsible for educating newly X-waivered physicians and APPs on the development of treatment plans and the clinical management of patients with Opioid Use Disorder.

Requirements:
  • Minimum 1-year direct experience providing MAT for Opioid Use Disorders in an outpatient setting
  • Certified Addictions Registered Nurse (CARN) certification or acquired within one year of starting position
  • Experience administering IM Sublocade and monitoring for post-injection adverse events
  • Experience in interpreting toxicology screening results, and developing a treatment plan that includes the assessment of these results.

Other Preferred Experience:
  • Knowledge of the social determinants of health that impact individuals with Opioid Use Disorder and community-based resources for this patient population
  • Experience with Crisis Intervention and de-escalation techniques
  • Knowledge of and experience with obtaining Prior Authorization for prescription medications

Responsibilities:
  • Must participate in all mandatory and relevant trainings offered by the Department of Public Health’s OBOT TTA
  • Must participate in all mandatory and relevant trainings offered by BSAS, including but not limited to ESM/Virtual Gateway training
  • Optional participation in trainings offered by the UMass Center for Integrated Primary Care including the Certificate of Intensive Training in Motivational Interviewing course
