This role provides leadership to Rehab Care Coordinators through training, coaching, and performance development, while also supporting order fulfillment, insurance and authorization coordination, and multiservice care pathways. The Lead Care Coordinator helps optimize clinic flow, reinforces clinical accuracy, and promotes a unified, accountable, and professional team culture.

The Lead Care Coordinator oversees treatment-floor operations, ensuring therapy excellence, documentation accuracy, order fulfillment, patient compliance, workflow efficiency, training, and team development.
Summary of Duties:
Supervises Rehab Care Coordinators, trains staff, oversees order completion, manages compliance workflows, supports billing readiness, ensures financial coverage integrity, and helps coordinate multi-service care pathways.
Duties & Essential Functions:
Clinical Workflow Leadership and Clinical Care
· Lead daily treatment-floor operations and maintain a leadership presence.
· Monitor clinical flow and adjust assignments to support efficiency.
· Reinforce therapy quality, safety, and adherence to provider directives.
· Assist and support Rehab Care Coordinator with delivery provider-directed care, therapies and DME dispensing and training.
Team Development, Coaching & Interviewing
· Participate in interviewing Care Coordinator candidates.
· Train, onboard, and mentor new Rehab Care Coordinators.
· Provide ongoing coaching and performance development.
· Participate in performance evaluations and development plans.
Care Plan Compliance & Outcome Ownership
· Monitor care plan adherence and escalate concerns when needed.
· Ensure therapy components and provider expectations are fulfilled.
· Support KPIs related to care plan completion, order fulfillment, and visit adherence.
Documentation, Order Completion & Clinical Accuracy
· Audit therapy documentation for accuracy and timeliness.
· Ensure provider-directed orders are communicated and fulfilled promptly.
· Support documentation workflows that minimize billing errors.
Insurance, Billing & Pre-Authorization Coordination (Clinical Oversight)
· Train Rehab Care Coordinators to recognize insurance or authorization obstacles.
· Monitor documentation for completeness that supports clean claims.
· Coordinate with Administrative Care Coordinators regarding authorization needs.
· Escalate authorization or eligibility concerns to the Practice Manager or Hub Manager.
· Ensure services are performed only when coverage or authorization is confirmed to protect revenue integrity.
Staffing, Scheduling & Flow Optimization
· Support Practice Manager or Hub Manager in staffing and clinical flow decisions.
· Identify clinic bottlenecks and recommend workflow improvements.
Team Collaboration & Interdepartmental Coordination
· Communicate consistently with Administrative Care Coordinators, Front Desk Supervisor, and providers.
· Support multi-service transitions across medical, imaging, rehab, and chiropractic care.

Experience Requirements
Minimum: 1 year as a Care Coordinator.
Preferred: 2–3 years clinical support experience with leadership responsibility.
Required Working hours are as followed:
- Sunday 8:30AM -1:00PM
- Monday: 730:AM - 1:00PM and 2:50PM - 7:30PM.
- Tuesday: 730:AM - 1:00PM and 2:50PM - 7:30PM.
- Wednesday: 730:AM - 1:00PM and 2:50PM - 7:30PM.
