• Schedule: Full-Time
• Setting: Field-Based / Home Care
• Patient Population: Pediatrics & Geriatrics
• Territory: Fairfield, Litchfield, Hartford & New Haven Counties

What You’ll Do

• Provide and oversee high-quality in-home nursing care
• Develop and manage individualized care plans
• Coordinate with physicians and interdisciplinary care teams
• Monitor, document, and report changes in patient condition
• Supervise and support home care staff
• Ensure patient safety, comfort, and independence at home

What We’re Looking For

• Active Connecticut RN license
• Minimum 2+ years RN experience (acute care required; home care preferred)
• Experience with pediatric and geriatric patients
• Clinical skills in:
  • Ventilator and tracheostomy care
  • G-tube/PEG tube management
  • IV infusions and wound care
• Current BLS certification
• Valid driver’s license
• Authorized to work in the U.S.
