About This Position

The Adult Services Nurse works under the direction of the Director of Residential Services Director, and is responsible for planning and coordination of medical services for adults who receive residential support(s) from FRA. The Nurse is responsible for expediting activities with medical personnel and vendors, discharge planning, level of care assessments and renewals, monitoring participant placement, training assisted living specialists, and working as a team member with ancillary agencies. The Adult Services Nurse assists in the development of individual plans of care and updating service plans. The Nurse is also expected to perform home visits, skills training, and direct care as needed.

DUTIES AND RESPONSIBILITIES:

  1. Monitors the safety, security, care and welfare of participants in the home. Provides direct assistance in identified problem areas.
  2. Conducts face-to-face visits and telephone contacts with assigned participants and/or guardians at least quarterly; more frequently as needed.
  3. Coordinates with the assigned care coordinator in preparing and submitting Medicaid waiver packets.
  4. Provides training to direct service staff and Residential Site Supervisors.
  5. Tracks critical deadlines, i.e. Medicaid and disability renewals, medical and specialty clinic appointments, and immunizations.
  6. Completes reports regarding contacts with participants and agencies.
  7. Participates in the development and implementation of Plans of Care (POC’s).
  8. Maintains close communication and collaboration with the Senior and Disabilities Services staff, hospital care coordinators and discharge planners, and others to advocate for needed services.
  9. Completes reports regarding contacts with families and agencies.
  10. Maintains participant records in accordance with State of Alaska standards of licensing for Nurses.
  11. Inputs participant information and case notes into electronic medical record, in accordance with agency procedures, state and federal requirements.
  12. Attends training seminars, in-service sessions, and staff meetings.
  13. Responsible for maintaining training compliance in accordance with FRA policies and procedures, state and federal regulations.
  14. Provides other support services and duties as necessary.

MINIMUM EDUCATION AND EXPERIENCE:

  1. State of Alaska Registered Nurse.
  2. Two years’ experience with adults who experience developmental disabilities preferred.
  3. Experience with Public Health nursing preferred.
  4. Must meet agency insurance criteria for operating agency and personal vehicles on agency business (21 y/o and licensed 3 years).

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

Demonstrates knowledge of and ability to apply principles of normalization and least restrictive environment, as well as the philosophy of the organization. Ability to train and provide technical assistance to direct care staff. Ability to problem solve and make decisions. Ability to effectively communicate with participants, families, other staff, personnel of other agencies and the general public. Ability to write plans of care and conduct assessments. Ability to work a flexible schedule. Must have reliable transportation and maintain a good driving record in order to conduct on-site visits. Must possess the knowledge and ability to utilize a computer and electronic medical record (EMR) system. Ability to learn new software and hardware applications.

LICENSES/CERTIFICATIONS REQUIRED:

Alaska Registered Nurse License

Alaska Driver’s License

Certification in Adult, Child-Infant CPR

Certification in First Aid

Safety-Care Certification

IMMUNIZATION REQUIRED:

Annual PPD Test

Hepatitis B immunization (or) Waiver

WORKING CONDITIONS:

Medium Work: Exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects and assist participants.

PHYSICAL ACTIVITIES:

Lifting: Raising objects from a lower to a higher position or moving objects horizontally from position-to-position.

Talking: Expressing or exchanging ideas by means of the spoken word. Those activities which demand detailed or important instructions spoken to other workers accurately, loudly or quickly.

Repetitive Motions: Substantial movements (motions) of the wrist, hands, and/or fingers, especially as related to typing notes and reports.

Driving: Minimum standards required by State Law (including license).

ENVIRONMENTAL CONDITIONS:

None: The worker is not substantially exposed to adverse environmental conditions (such as typical office or administrative work).
