Job Summary

The Patient Care Coordinator Manager is accountable for the management of the PCC staff that that work in partnership with physicians, facilities, discharge planners, and clinic staff to promote home healthcare, hospice and home medical equipment services in order to provide a smooth transition from the acute, skilled nursing facility and clinic setting to the home across the Avera@Home footprint.

Education and/or Experience

• Bachelor's degree in nursing preferred.
• Graduate from a CCNE or NLN accredited nursing program is strongly preferred.
• Must demonstrate sound communication and sales skills.
• Training and experience in health care related field, home health or hospice preferred.
• Previous supervisory or management experience preferred.

Licensure/Certification/Registration

• Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure.
• Licensed as a Registered Nurse in the State of Minnesota may be required depending on the service area. If required, must obtain within 90 days of being placed in the position or of the requirement.
• This position requires a valid driver’s license and that the employee is insurable by the Avera Health automobile liability insurance carrier.
