About This Position

Job Overview:

As part of RWJBarnabas Health's Population Health Department, you will coordinate care for patients transitioning from our hospitals to post-acute care facilities in our Preferred Post-Acute Network. This role focuses on optimizing patient experience, safety, and value-based care across skilled nursing facilities, inpatient rehabilitation, and home health services. You will collaborate closely with post-acute partners, conduct performance reviews, analyze quality data, and facilitate communication to enhance care quality and efficiency.

Qualifications:

Required:

  • Associate degree in a healthcare-related field

  • Experience in data-driven performance improvement strategies

  • Knowledge of healthcare economics and regulations (skilled nursing, home health, BPCI A, MSSP/ACOs, CMS Waiver Programs)

  • Strong organizational and interpersonal skills

  • Ability to travel across NJ to post-acute and hospital sites

  • Proficiency in Microsoft Office and virtual conferencing tools

Preferred:

  • 2+ years of operational or clinical leadership in skilled Nursing Industry
  • Certified Medical Assistant and/or Licensed Practical Nurse preferred

Scheduling Requirements:

  • Must have the ability to travel to post-acute sites and hospital sites throughout the state of New Jersey.

Essential Functions:

  • Oversee the performance of 56 skilled nursing facilities within the RWJBH network, managing over $60 million in post-acute care spending.

  • Serve as a liaison between post-acute facilities and RWJBH to foster new and existing partnerships.

  • Conduct quarterly performance reviews and report on value-based payment models to identify and address performance gaps.

  • Collaborate with care coordinators to ensure seamless discharge planning and continuity of care for patients.

  • Engage regularly with hospital leadership and value-based care stakeholders to monitor SNF performance and optimize patient outcomes.

  • Review network composition annually and recommend changes for improved collaboration and performance.

  • Report regularly to leadership to enhance visibility on network performance.
