Licensed Alcohol and Drug Counselor - Opiod Addiction Response at Cheshire Medical Center – Keene, New Hampshire

PURPOSE OF POSITION: Provides behavioral health triage assessments and evaluations, maintains an active patient caseload, monitors psychiatric and substance misuse symptoms and other measures to track outcomes, actively facilitates referrals to address behavioral health and social determinants of health needs.

Responsibilities:

1. All other duties as assigned.
2. Demonstrate patience, courtesy, and compassion when interacting with visitors, patients, employees, community partners and others.
3. Responsible for working collaboratively with other providers and to make dispositions that are appropriate, are clinically sound and ensure the safety and well-being of the patient.
4. Demonstrate competency and/or participate in training as required for licensure and professional growth.
5. Ensure data collection, clinical documentation, and outcome reporting is met in compliance with organization, insurance requirements, and funder.
   1. Completion of the Government Performance and Results Act (GPRA) interview; collecting and documenting attempts to collect client-level data at multiple intervals including at intake, 3 months post intake, 6 months post intake and upon discharge.
   2. Ongoing follow-up and support of clients engaged in services
   3. Ongoing assessment in collaboration or consultation with the client’s external service provider(s) of necessary support services to address needs identified in the evaluation or by the client’s service provider that may create barriers to the client entering and/or maintaining treatment and/or recovery.
   4. Facilitate referrals to substance use disorder treatment and recovery support and other health and social services as deemed necessary
6. Care Coordination:
   1. Obtain releases of information requisite to the administration of care including disclosure of protected health information.
   2. Ensure client compliance with the policies and procedures for the delivery of medically assisted treatment at the Doorway.
   3. Identify clients’ substance use disorder(s) and develop goals and treatment plans.
   4. Maintain a caseload of clients and provide counseling and therapy to these clients either individually or in group.

   1. Development of a treatment plan in collaboration with the client based on the clinical evaluation.
   2. Evaluation of all American Society of Addiction Medicine Criteria (ASAM, October 2013), domains and facilitate and/or provide care based on ASAM criteria.
   3. Crisis intervention and stabilization which ensures that individuals in an acute OUD related crisis that require immediate, non-emergency intervention are provided with crisis intervention counseling services by a licensed clinician.

REQUIREMENTS OF POSITION:

• Experience: Minimum 2 years clinical experience in a relevant setting.
• Licensure: Applicants must either be licensed in the State of New Hampshire as a LADC, MLADC, LICSW, LCMHC, or working towards a master’s level license.
• Education: Master’s Degree (Preferred) in Psychology, Social Work, Counseling, Counseling Psychology, or a related clinical field from an accredited college or university plus relevant experience in providing crisis assessments, brief treatment, and crisis counseling. Preference given to a degree that is certifiable or licensable in N.H. or post Master’s employment in crisis intervention, brief treatment, and the provision of intake assessments.

Compensation

• Minimum Pay: $24.51
• Maximum Pay: $36.76
