Every 3rd-4th weekend

The Rush Memorial Hospital Family Nurse Practitioner (FNP) leads with compassion and customer service. Patient-centered care and service are their top priorities. At Rush Memorial Hospital, Advanced Practice Providers are highly respected and considered an integral part of the health care team. This includes being a part of the Medical Staff and decision making. The Rush Memorial Hospital Family Nurse Practitioner (FNP) is a professional registered nurse with advanced education, training and certification in the primary care of the family.

The FNP is responsible for providing primary care, including wellnesses, chronic disease management and acute car, to patients as directed by clinical protocols. As per State of Indiana guidelines, the nurse practitioner utilizes a collaborative practice agreement with a physician. Family Nurse Practitioners are utilized in the outpatient clinics of Rush Memorial Hospital, which include Walk-in Care, Health Care Associates (Mobile Clinic), Milroy Primary Care, RMH Pediatrics and potentially, RMH Specialists.

Family Nurse Practitioners have assigned positions but may help in other clinics on an as needed basis. Minimal on-call (via phone only) may be part of this position, approximately every 8 weeks, depending on assignment. Weekend mornings may be a part of this position, approximately every 4 weeks depending on assignment

Requirements:

Knowledge & Experience

Skills-

  • Substantial interpersonal skills necessary to instruct patients and to collaborate with health team members.
  • Demonstrates physical and emotional stability necessary to function professionally.
  • Effective communication skills in verbal, non-verbal, written and electronic media.
  • Role model of professionalism in nursing practice.
  • Ability to work independently with collaboration.
  • Strong documentation and computer skills.

Knowledge-

  • Professional knowledge of nursing theory and practice.
  • Extensive knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology and patient care management.

Education-

  • Current Indiana RN and APN nursing license with renewal required.
  • A minimum of a Master of Science Degree in Nursing is required.
  • Certification from an approved accreditation board
  • Maintains CEU’s for certification renewal.
  • CPR certified.

Experience-

  • Minimum of three years of staff nursing experience required.
  • One to three years of experience as nurse practitioner preferred.

Authority and Accountability

  • Good working knowledge of hospital policies and procedures.
  • Extensive exposure to confidential information or matters which improper handling could result in poor public image of the hospital.
  • Frequent contact with various people or groups within and outside of the hospital including employees, board members, physicians, visitors and outside agency personnel.
  • Must be able to attend meetings at night when requested.
  • Must be able to work with, communicate with, and properly manage various types of people.

Aptitudes

  • Clerical competency is required to copy data from rough notes and to prepare notes accurately.

ACCOUNTABILITY

  • Reports to the Director of the respective department assigned and the VP of Provider Services.

QUALITY ASSURANCE-KEY RESPONSIBILITIES

  • 1. Highest Clinical Care
  • Obtains relevant health history and comprehensive physical exam, preventive screening, and identification of medical risk factors.
  • Evaluates assessment data and establishes diagnoses.
  • Differentiates patient care situations which require immediate and non-immediate actions and responds accordingly.
  • Orders and evaluates appropriate diagnostic studies.
  • Performs and monitors therapeutic procedures.
  • Assesses and/or manages follow-up plans.
  • Acts as integral component of on-going case management and added assistance in daily operational flow when not performing direct patient care.
  • Records and documents assessment data, interventions, results, and patient outcomes.
  • Provides comfort, privacy, and confidentiality in a kind, respectful, nonjudgmental manner to patient and family/support person.
  • Demonstrates ability to work effectively with patients, employers, and with professional and supportive personnel who provide patient care.
  • Participates in all third party payer quality programs and works to meet those quality goals
  • Takes appropriate action in problem situations to offset adverse effects.
  • Educates patients and/or their employers in order to promote wellness, prevent health problems, maintain current health and intervene in acute or chronic illness.
  • Documents teaching and patient instructions for follow-up.
  • 2. Delivers the administrative and communication needs of the organization and patients effectively and timely
  • Facilitates and implements systems which promote and maintain effective communication and collaboration with inter/intra hospital physicians, nurses, and other members of the health care team in addition to the employer.
  • Maintains appropriate records and documentation of clinical and educational activities.
  • Provides comfort, privacy, and confidentiality in a kind, respectful, nonjudgmental manner to patient and family/support person.
  • Collaborates with physicians as needed, in the development of clinical protocols/procedures for the delivery of care.
  • Demonstrates ability to work effectively with patients, employers, and with professional and supportive personnel who provide patient care.
  • Recognizes and uses appropriate channels to communicate stressful patient care/performance situations that arise with physicians, employers, and other staff members.
  • Acts as integral component of on-going case management and added assistance in daily operational flow when not performing direct patient care.
  • Works collaboratively with the department director to achieve patient and department goals.
  • 3. Quality/Customer Service
  • Patient and staff safety is of the utmost importance and identifies the need for and participates in the collection and interpretation of relevant data on issues/ problems relating to the CQI process.
  • Works diligently toward achieving the highest scores on managed care quality measures.
  • Performs care with low patient wait times as a priority for evaluation, answering messages and providing results.
  • High concern for internal and external customer service.
  • 4. Continually increases clinical knowledge and provides education to patients and staff
  • Acts as a resources to hospital staff in providing formal and informal instruction to improve clinical skills, enhance knowledge base and improve patient care.
  • Maintains and enhances a current level of knowledge relative to professional practice, as well as continuing education requirements necessary for certification.
  • Accepts self-responsibility for advancing skills and continuing education by:
  • Setting realistic and measurable goals for individual development needs based on personal, peer and supervisory evaluations of strengths and learning needs.

  • Attends 70% of scheduled staff meetings for primary work area during the year.

  • Conducts self in professional manner acting as a role model while practicing and promoting effective guest relations.
  • Provides proper notification for absence and tardiness and is punctual for start of scheduled work time.
  • Complies with hospital policies and procedures.
  • Completes projects and assignments on a timely basis.
  • Participates in emergency procedures, fire drills, evacuation procedures and internal/external disaster plans, procedures in accordance with departmental responsibilities, and mock codes.
  • Participates in committees to improve patient care, the patient experience or employment relation.

CONTINUOUS QUALITY IMPROVEMENT

  • Participates in the Continuous Quality Improvement Program under the direction of the Chief Executive Officer to meet stated goals and objectives of the Continuous Quality Improvement Program by:
  • Brings potential problems to the VP of Provider Services attention.
  • Assists in data collection.
  • Suggests solutions to problems.
  • Emphasizes, supports and is committed to CQI practices and processes.
  • Takes action to assure CQI within areas of responsibility.
