About the Role – Housing Specialist
The Housing Specialist is responsible for coordinating housing-related services and supports for members experiencing housing instability, homelessness, or financial hardship. This position assists individuals with housing navigation, rental assistance coordination, landlord communication, and connection to community housing resources through StrongSteps Behavioral Health programs and Health-Related Social Needs (HRSN) services.
The Housing Specialist works closely with Care Managers, Intake staff, community providers, and landlords to help members secure and maintain stable housing while ensuring all required documentation meets agency and compliance standards.
Responsibilities- Assist members with housing navigation and housing stabilization services
- Coordinate rental assistance requests and required housing documentation
- Communicate with landlords, property managers, and housing providers
- Verify leases, ledgers, balances owed, and supporting documents
- Connect members to housing resources and community programs
- Maintain accurate and timely case documentation
- Track housing referrals, applications, and service outcomes
- Support members with housing-related barriers and follow-up coordination
- Ensure housing files meet Medicaid, HRSN, and agency compliance requirements
- Collaborate with care management and administrative staff regarding member needs
- Strong communication and organizational skills
- Experience in housing services, social services, behavioral health, or case management preferred
- Ability to manage multiple cases and maintain accurate documentation
- Comfortable working with vulnerable populations and individuals experiencing housing insecurity
- Basic computer and documentation skills required
- Knowledge of housing resources, landlord coordination, or HRSN services preferred
- Full-Time Position
- Hourly, Non-Exempt
- Starting Pay: $25/hour
