We are currently seeking an experienced:
CENTRAL INTAKE SCHEDULER
Full Time
JOB SUMMARY:
- Coordinate and schedule all admission and routine visits for hospice, home health, and palliative patients, with priority given to urgent and same-day requests.
- Maintain proactive and ongoing communication with referral sources, marketing team, field clinicians, intake staff, and patient families to ensure clarity, consistency, and timely follow-up.
- Accurately input and update all scheduling activities within the EMR to support data integrity and meet regulatory standards.
- Anticipate, identify, and resolve scheduling conflicts or delays in the admission process to prevent service disruptions.
- Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure appropriate staff assignments.
- Maintain clear, organized documentation of all scheduled admissions and completed visits for tracking and reporting purposes.
- Perform additional duties as assigned to support departmental goals and patient care outcomes.
JOB REQUIREMENTS:
- High school diploma or equivalent required
- Associate degree or healthcare-related coursework preferred
- Experience in Microsoft 365, medical scheduling, admissions coordination, or hospice/home health preferred. Knowledge of healthcare terminology and processes.
- Strong organizational and time management skills, with the ability to prioritize tasks, manage multiple schedules, and meet tight deadlines.
- Detail Oriented with a high level of accuracy.
- Problem-solving skills to anticipate and address scheduling conflicts and delays
